For seven years, CDBG funding from the City of Santa Ana has allowed us to focus on providing wraparound legal services to dozens of Santa Anaresidentsin an effort to help themobtainlong-term safety and well-being. Here is one example of how this grant hasimpacted the lives of your constituents: earlier this year, CLA SoCal was calledby a Santa Ana resident who was in an abusive relationship, andwho was trying to get a restraining orderon her own. Unfortunately, her abuserand his attorneyhadsuccessfully stalledthe processfor months. We were able to helpour client file the paperwork that led to a hearing, where we fiercely represented her. Because of the support our CDBG- funded staff were able to provide, our client was able to resolve astressful and dangeroussix-month process and obtain a permanentrestraining order. Good morning Friends at the City of Santa Ana, Late last year, MOMS Orange County had the opportunity to submit a Community Development Block Grant application for consideration to the City of Santa Ana. We were recently invited to speak on behalf of our application during the public comment portion of today's city council meeting. If, given the many high priority items that the council must attend to, there is not time for us to speak this evening, we respectfully request that the following comments be forwarded to the council in support of MOMS' application under item #29: Good evening Mayor and Council Members, First and foremost, we, the staff, Board, and leadership of MOMS extend our thanks for your consideration of the application before you and for the many years of partnership we've had working together to give Santa Ana moms and babies the healthiest starts possible. Since 1992, it has been our pleasure to work with thousands of local moms, dads, and their little ones, giving them the knowledge, skills, screenings, and support they need to thrive. We have submitted an application to continue this storied partnership into the next Fiscal Year, so that our Maternal Child Health Coordination program can continue to support moms with flexible in-home and virtual visits to ensure that each participant is equipped with the resources and information they need beginning in pregnancy and lasting through their baby's first year of life. Each year, MOMS serves approximately 750 Santa Ana mothers and infants across roughly 2,500 in- home visits. Although our clients face many barriers to care and even more to establishing proof of income and legal residency, we are able to obtain qualifying documentation from roughly 150 low- income participants each year which, for intents of measuring CDBG progress, is our proposed service goal. Our services begin in pregnancy when a participant reaches out to us either by phone, online, or by way of referral from their doctor or community health clinic. She is then paired with a Maternal Child Health Worker (MCHW) operating under RN supervision from MOMS who meets with her on a monthly basis, developing a birth plan, conducting mental health screenings, and providing bilingual resource navigation (available in Spanish and Vietnamese) to ensure that expecting moms receive proper early prenatal care. Where our nurses identify potential areas of concern, they work with the family and MCHW to identify resources to resolve or mitigate risks, including, where need be, referrals to on-site licensed mental health counseling courtesy of MOMS. After the baby arrives, we collect data on birth outcomes and continue working with mom and baby alike to promote healthy development, support maternal health, and measure progress toward critical milestones including physical and psychosocial development as well as healthcare access such as completion of immunizations and well-baby visits. In 2023, we were proud to find that of the babies 1 served through our program, 92% were developmentally on-track at closure, 93% had completed doctor-recommended vaccination schedules, and 100% had a medical home with a pediatrician upon graduating. As of early 2024, the Maternal Child Health Coordination program has rendered its services in partnership with CalOptima through its Community Health Worker and Enhanced Care Management initiatives under CalAIM. Given that 90% of our participants are either enrolled in or eligible for CalOptima plans, these initiatives have significantly expanded our capacity to provide care coordination among healthcare providers and other community-based organizations to ensure that clients' immediate health needs are met and that social determinants of health such as housing needs are addressed. Bullying has profound and long-lasting effects on our youth, impacting their academic performance, mental health, and overall well-being. By engaging teens in crafting policy documents, building leadership and professional soft skills, and participating in intensive training sessions, TELL equips our youth with the necessary tools to combat bullying effectively and create a safer environment for all. Furthermore, the integration of technology and natural resource exploration, made possible through partnerships with UCI and the Irvine Ranch Conservancy, represents a groundbreaking approach to youth empowerment. By exposing our teens to AI literacy curriculum and outdoor excursions, TELL not only enhances their academic and technological proficiency but also fosters a deeper connection to their surroundings, promoting community awareness and engagement. Dear esteemed Mayor and City Council, The Delhi Center serves over 1300 individuals weekly who rely on us for critical basic needs such as food and diapers. Yet, “over the next few months, the OC Hunger Alliance (comprised of Second Harvest Food Bank, Community Action Partnership, and Abound Food Care) expects a significant and serious decrease in food and nutritional security due to factors including the end of pandemic related food allotments, double and triple home heating bulks, increasing food prices, and continuing supply chain challenges”1. In addition, as the 2024 – 2025 state budget looms, the pivotal resource of diapers may also be at-risk 2. These are but two reasons why the Delhi Center’s TELL (Teens Engaged in Leadership and Learning) Program is instrumental. Let me explain. The success of the TELL program is based on a transformative approach that empowers teens to navigate complex issues. The program's unique blend of education, leadership, and self-advocacy empowers participants to be active contributors to their community's well-being, exemplifying the potential for positive change. WE ARE NOW TAKING THIS PROVEN MODEL AND APPLYING IT TO THE ADULTS IN OUR COMMUNTY. We call this Empower360 – empowering individuals and transforming their well-being. Like TELL, Empower360 centers on collective impact initiatives and strategic community partnerships. Program execution is based on a collaborative framework that promotes educational and health equity through targeted strategies with local schools, nonprofits, and the City Library. By engaging key stakeholders and collectively addressing community needs, the Delhi Center seeks to change the trajectory of vulnerable lives. Thus, we are convinced we will begin the process of ending poverty in at least 100 families by December 2026.
