SECOND AMENDMENT TO
CONSULTANT AGREEMENT
THIS SECOND AMENDMENT TO CONSULTANT AGREEMENT is entered
into this lOth day of August, 2005, by and between Leanna Forcucci-Herron
("Consultant") and the City of Santa Ana, a charter city and municipal corporation of the
State of California ("City").
RECITALS:
A. The parties entered into Agreement N-2004-045, dated May 5, 2004, (hereinafter
"said Agreement") by which Consultant has instructed Hip Hop I Jazz dance classes
through the City's leisure class program.
B. Consultant has formed a general partnership and desires that said Agreement be
amended to reflect the partnership is providing the services rather than the individual.
In accordance with the terms and conditions of said Agreement, the parties wish to
renew said Agreement for an additional one-year period.
WHEREFORE, in consideration of the covenants contained in said Agreement, and
subject to all the terms and conditions of said Agreement, as previously amended, except
those amended in this Second Amendment to Consultant Agreement, the parties agree as
follows:
I. The parties to said Agreement shall be amended to reflect that Consultant providing
services is Cathleen Forcucci Dance Academy, a California general partnership.
2. Except as herein amended, all terms and conditions of said Agreement shall remain
in full force and effect.

IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to
Consultant Agreement on the date and year first written above.
