ginning and ending dates of the periods to which the
salaries or wages apply.
If there is i.nsufTicient space for the informe.tion under any item, enter
"over" and show the information on the reverse sj,de of the card, number-
ing it the same as 'the item on the clJrd.
Three copies of F1l3 and two of Fl1l~, listed in items A and B, and one
copy of each of F1l5u... listed in item C, are to be forwarded directly to the Retire-
mont Office in Sacramento, which in t.urn,vill have the contre.ct executed by the
consulting actuary and one copy returned to you, after approval by the BOard of
Administre.tion.

If this date is not suitable to you, will you please let us know as soon as possible.
If. it~is acceptable, then the service for three years prior to that date will be
used on cards form l15b.
