HomeMy WebLinkAboutSANTA ANA EMPOWERMENT CORPORATION 3D - 2006INSURANCE NOT RE(~UIRED A-2006-184-01 WORK MAY PROCEED CLERK OF COUNCIL DATE; , fc - ~ -L,Y C c~R `~) FIFTH AMENDMENT TO AGREEMENT BETWEEN THE SANTA ANA EMPOWERMENT CORPORATION L s<_} (.~~rr~c,nc~ Nz AND THE SANTA ANA W/O/R/K CENTER FOR USE OF EMPOWERMENT ZONE FUNDS THIS FIFTH AMENDMENT TO AGREEMENT ("Amendment") is entered into this 4th day of ~A ril 2007, by and between the Santa Ana Empowerment Corporation, a California non-profit public benefit corporation ("SAEC") and the City of Santa Ana, a charter city and municipal corporation of the State of California, on behalf of the Santa Ana W/O/R/K Center ("Subrecipient"). Recitals: A. The parties entered into an Agreement dated July 1, 2003, (hereinafter "said Agreement") by which SAEC granted to Subrecipient, Empowerment Zone ("FEZ") funds in the amount of $930,000.00, granted to SAEC from the United States Department of Housing and Urban Development. B. On April 5, 2004, the parties entered into a first amendment to said Agreement, which added $12,000 in first year funds to the Subrecipient for marketing and outreach efforts. C. On or about June 23, 2004, the parties entered into a second amendment of said Agreement, which increased funding by $980,000.00 to provide for a second year of programs. D. On or about July 18, 2005, the parties entered into a third amendment of said Agreement, which increased funding by $980,000.00 to provide for a third year of programs. E. In January of 2007, the parties entered into a fourth amendment of said Agreement, which increased funding by $980,000.00 to provide for a fourth year of programs for the 2006-2007 program year. F. The parties now wish to revise the composition of the enrollment number of 800 new Federal Empowerment Zone residents to allow for said enrollment number be comprised of youth and/or adults and remove the specific limitations for youth and adult enrollment numbers, respectively, as specified in greater detail in Exhibit A to this Amendment. Wherefore, in consideration of the covenants contained in said Agreement, and subject to all the terms and conditions of said Agreement, as previously amended, except those amended in this Fifth Amendment to Agreement, the parties agree as follows: 1. Exhibit A to the Agreement shall be deleted in its entirety and replaced with the Exhibit A attached hereto. 2. Except as hereinabove amended, all terms and conditions of said Agreement shall remain in full force and effect. Q City Manager APPROVED AS TO FORM: JOSEPH W.FLETCHER City Attorney ,~_ _ By: Be j in Kau an C i f Assistant City Attorney Federal Empowerment Zone SERVICE NAVIGATOR SCOPE OF WORK Phase IV 2006 - 2007 The Service Navigator (Santa Ana WORK Center), under Phase IV of the Daisy Wheel Network Program will be responsible for delivering the following: Serve 800 new FEZ residents under the same scope of services mentioned in the original proposal. • 250 new youth (ages 16-18) • 550 new adults 2. 40% (320) of the 800 residents served will be provided with WEX, receive OJT, retention services, attend ESL classes, provided with skill upgrades, apprenticeship and/or internships. 3. 30% (240) of the 800 residents served will be placed in unsubsidized jobs 4. 50% (400) of the 800 residents served will be provided with Additional Services In addition to the above services, outcome measures for youth would be: 1. During the school gear, youth would be required to work 10 hrs/wk PLUS full-time classroom instruction; 2. During the summer, there would be two outcome options: 1) work 20 hrs/wk plus part-time classroom instruction; or 2) work 40 hrs/wk; Year-round, youth will be expected to stay in school; participate in career preparation workshops, character development, job search, resume workshops, interviewing techniques, credit counseling & money management, financial aid assistance info, and/or life skills workshops; attend college or higher ed. informational workshops, scholarship information sessions, plus other workshops. EXHIBIT A March 23, 2007 Deborah Sanchez SAEC Manager 20 Civic Center Plaza Santa Ana, CA 92701 SUBJECT: Contract Modification Request for Daisy Wheel Service Navigator Program Grantee Name: Santa Ana WORK Center Dear Deborah, I am writing to request the SAEC Budget & Program Task Force Committee approval for a contract modification on our Daisy Wheel Service Navigator grant. This request would take effect April 9, 2007 through June 31, 2007. The contract modification would consist of changing enrollment numbers for the youth and adult component. The demand for youth services (ages 16-18) has not been as high as we anticipated. We do, however, have a great demand for adult services. In light of this, we are requesting that the 800 enrollment number be interchangeable between adult and/or youth. This contract modification request will afford us to serve adults who are currently on a waiting list, and the opportunity to meet grant numbers, specifically when it comes to enrollments and placements. This requested grant modification would only change the grant from a specific enrollment number for youth and adults, to where either youth or adults could participate, depending on the demand of services. We would still enroll 800 FEZ residents. The enrollment modification will give us and our sub-contractors an opportunity to meet enrollment numbers.
