On Friday, June 25th, 2021, at approximately 1124 hours, Santa Ana Police Department
Correctional Supervisor Monreal contacted the Watch Commander, J. Viramontes, regarding
a U.S. Marshalls Inmate found unresponsive in his cell, located in Module 4C, cell 17.
A patrol officer was requested to respond to follow the inmate, Darryle Marcell Samuel, Sr, if
he were to be transported to the hospital by Orange County Fire Authority via CARE
ambulance.
Medical care was provided for Samuel until relieved by Orange County Fire Authority, who
pronounced Samuel deceased at 1150 hrs. Samuel was not transported from the scene.
The Santa Ana Police Department Homicide Detectives and Investigators from the Orange
County District Attorney Special Assignments Section responded to investigate.

Weather & Lighting:
Upon my arrival within the housing unit on Friday, June 25th, 2021, at approximately 1435
hours, the module was approx. 72 degrees, and lighting was provided by LED lighting in the
module as well as natural light through the various windows.
Scene:
This incident occurred in Module 4C within the Santa Ana Detention Facility at 62 Civic
Center Plaza. The Santa Ana Detention Facility is located adjacent to the Santa Ana Police
Department. The United States Marshalls¶Service has a contract to house federal inmates
at the Santa Ana Detention Facility. Module 4C, located on the 4th floor of the jail, is a direct
supervision housing module consisting of 16 cells on the upper and lower tiers.
A direct supervision module has an officer desk in the dayroom area, located on the 1st
floor. There is a control panel that allows for the remote unlocking of individual cell doors or
all doors at once. When the door is released, the door lock releases and allows the door to
open slightly.
Each of the cells are designed to house 2 inmates, however some cells, including cell 17,
only had 1 person assigned for housing.
Cells 1-16 are located on the lower tier, and 17-32 are located on the upper tier.
There is a dayroom with access to a multi-purpose room as well as a recreation yard on the
1st floor. There is a single staircase leading to the upper tier.
Cell 17 is in the northeast corner of the module, with a south facing door that opens outward
from right to left.
The ³C ´is on the front of the door, as well as on a plastic marker on the wall to the left of

the door. The door has a narrow, vertical window as well as a vented plate to allow for
communication.
The cell is ¶deep and ¶´wide at its¶widest point. There are bunk beds at the back
(north end) of the cell, a metal table top affixed to the west wall ´from the floor) with a
metal shelf ´above the table top.
There is an approx. ´space between the shelf and wall. A white bedsheet was tied around
the shelf and extended down below the table top.
A metal toilet and a sink were located on the west side of the cell, between the entry door
and the table top.
The decedent, Darryle Samuel, was lying in a supine position with his head under the table
top. His head was facing west and his feet were facing east. He was wearing an orange
colored do-rag, tan sweatpants with a cut up the left leg, orange socks, and orange/white
briefs.
He had medical devices still attached to him, including a disposable resuscitator bag,
defibrillator pads, and an IV.
Samuels¶inmate wristband was removed and located on top of his table top. Samuel had
numerous tattoos, including ³COMPTON´across his abdomen, ³KPCC´on his right bicep,
³KP´on his chest, and ³CRIP´above his left breast.
NARRATIVE
Friday, June 25, 2021:
1155 hours:
I was contacted by Detective Sergeant M. Sarabia #2565 regarding a death investigation
involving a United States Marshall Service inmate currently housed at the Santa Ana
Detention Facility.6JW 6DUDELD UHTXHVWHG , UHVSRQG WR DVVLVW ZLWK WKLV GHDWK LQYHVWLJDWLRQ
1300 hours:
I arrived at the Santa Ana Police Department and met with other detectives in the homicide
section, where we awaited the arrival of investigators from the Orange County District
Attorney Special Investigations Section.
1345 hours:
Sgt Sarabia provided a pre-briefing of the details known to him at the time. Sergeant
Sarabia stated this morning, at approximately 1125 hours, Santa Ana Jail Supervisor M.
Monreal #3181 contacted the Santa Ana Police Department Watch Commander and
UHSRUWHG WKDW DQ LQPDWH ZDV GRZQ DV D UHVXOW RI DQ DSSDUHQW VXLFLGH DWWHPSW 2UDQJH
County Fire Authority Medics responded to the jail and a police unit was being requested to

was pronounced deceased and was not being transported to the hospital.

The deceased inmate was subsequently identified as Darryle Marcell Samuel (DOB

I was assigned to document the scene and was paired to work with Orange County District
Attorney¶s Office Special Assignments Investigator Dave Melnyk.
1415 hours:
I responded to the Santa Ana Jail with personnel from the Santa Ana Police Homicide

from the Santa Ana Police Department, Orange County District Attorney¶s Office, and
Orange County Sheriff-Coroner¶s Department.
Supervisor Monreal informed us that this morning, at approximately 1119 hours, Santa Ana
Jail Correctional Officer E. Gonzalez #3366 radioed to jail staff for medics and additional
FRUUHFWLRQDO VWDII WR UHVSRQG WR 0RGXOH & 7KLV 0RGXOH ZDV FRPSULVHG RI WKLUW\WZR FHOOV
capable of holding two inmates each and was designated as a protective custody unit, with
inmates who were either cooperators with law enforcement or charged with sex-related
offenses.
Supervisor Monreal indicated that she responded to the Module at approximately 1120
KRXUV 7KH DVVLVWDQFH ZDV EHLQJ UHTXHVWHG E\ &RUUHFWLRQDO 2IILFHU *RQ]DOH] IRU WKH LQPDWH

cell attempting to render medical aid to the inmate in Cell 17.

Authority (OCFA) personnel from Station 76 arrived on scene and assumed medical aid of

Fire Captain Robert Richardson #5760 informed Supervisor Monreal that Samuel was
deceased and would not be transported from the scene.
Supervisor Monreal indicated that at approximately 1152 hours the doors to Cell 17 were
closed and the cell was secured with no additional persons accessing the cell.
Supervisor Monreal advised us that inmate Samuel came out of his cell to the day room of

approximately 0930 hours, Correctional Officer S. Tolone #3687 conducted a welfare check
on Samuel¶V FHOO DQG GRFXPHQWHG WKH FKHFN LQ D ORJ $W DSSUR[LPDWHO\ KRXUV WZR
inmate workers named ³ ´and ³ ´were going from cell to cell cleaning and
providing deliveries and then alerted custody staff that there was an inmate hanging in the

Preliminary indications suggested that Samuel committed suicide in his cell this morning
through self-inflicted hanging.

Santa Ana Jail Correctional Officer D. Godinez #3694 was assigned to complete a crime

1506 hours:
Deputy Coroner Babcock began her physical body examination of Samuel. She noted a
slight line on the right side of his neck, but an absence of any defined ligature marking. She
also did not observe any petechiae in either eye of Samuel.
1615 hours:
At the conclusion of the preliminary examination of Samuel, Cary Franco and Draydon
Dehart from Traditional Funeral Services arrived ato transport Samuel to the Orange County
Coroner¶s Office.
1619 hours:
The physical inspection of Samuel¶s body was completed and he was placed in a white body
bag, with seal # 0195019 affixed.
1620 hours:
Samuel was removed from Module 4C.
The Deputy Coroner as well as Orange County Crime Lab personnel also left the scene.
1635 hours:
Due to the number of inmates housed in Module 4C, District Attorney Investigator D. Melnyk
and I assisted other investigators with interviews of some of the inmates housed within the
module.
We conducted interviews in the multipurpose room. The interviews were audio recorded by
DA Investigator D. Melnyk and retained by him to be booked as evidence.

Tuesday, June 29, 2021, 0910 hours:
On the above date and time, I attended the post mortem examination of twenty-eight year old Samuel
Darryle Marcell Sr. The examination was conducted at the Orange County Sheriff-Coroner¶s Office located
at 1071 W. Santa Ana Boulevard in Santa Ana. Doctor Kim was the pathologist conducting the examination
under Orange County Sheriff-Coroner Department Case number 21-03893-BK and Orange County Sheriff-
Coroner Department Forensic number 21-48216. In addition, present during the examination were Forensic
Assistant J. Savino, Forensic Scientist C. Stark, and Forensic Specialist K. Leon. For further information
regarding the examination performed, refer to the Orange County Sheriff-Coroner¶s report under case
number 21- 03893 - BK and FR 21-048216.
Marcell¶s body was removed from a white body bag and placed on the examination table in a supine
position. I observed Marcell¶s body to be nude with a toe tag on his right foot and medical paraphernalia. I
observed Marcell to have numerous tattoo¶s on his body. He had ³Compton´on his left upper arm, ³´
underneath his right eye, ³´underneath his left eye, and above his left eye. Marcell had several tattoos on
his chest and neck area.
Dr. Kim concluded the post mortem examination of Marcell and stated his body did not have any signs of
trauma. Dr. Kim noticed shallow marks on both sides of Marcell¶V QHFN ZKLFK DSSOLHG SUHVVXUH WR WKH
carotid which caused the lack of oxygen to the brain. Dr. Kim stated the cause of death was ligature
hanging and she determined it was a suicide. For further details, refer to Dr. Kim¶s report under Coroner
Case number 21 - 03893 ±BK.
This concluded my observation of the post mortem examination.

Santa Ana Police Department Homicide Sergeant M. Sarabia #2565 notified me of an in-custody death that
had occurred at the Santa Ana Jail. Sergeant Sarabia requested that I respond to the Santa Ana Police
Department in order to assist with the investigation.
1345 hours:
I responded to the Santa Ana Police Department and was provided a preliminary briefing of the incident by
Sergeant Sarabia. Present at this briefing were personnel from the Santa Ana Police Homicide Section and
Orange County District Attorney Special Assignments Unit.
Sergeant Sarabia informed us that this morning, at approximately 1125 hours, Santa Ana Jail Supervisor M.
Monreal #3181 contacted the Santa Ana Police Department Watch Commander and reported that an
inmate was down as a result of a suicide attempt. Orange County Fire Authority Medics were responding to
the jail and a police unit was being requested to follow the medics with the inmate to the hospital. At
approximately 1154 hours, the inmate was pronounced deceased and was not being transported to the
hospital.
The deceased inmate was subsequently identified as Darryle Marcell Samuel (DOB Samuel
was being housed in cell ³C17´by himself. Samuel was a federal US Marshals inmate who was presently
acting as an informant on a homicide case.
1415 hours:
I responded to the Santa Ana Jail with personnel from the Santa Ana Police Homicide Section and Orange
County District Attorney Special Assignments Unit. We were subsequently escorted to 4th Floor Module 4
C. Supervisor Monreal then conducted a briefing regarding the circumstances of the incident. Present at
this briefing were personnel from the Santa Ana Police Department, Orange County District Attorney ¶s
Office, and Orange County Sheriff-Coroner¶s Department.
Supervisor Monreal informed us that this morning, at approximately 1119 hours, Santa Ana Jail Correctional

Module 4 C. This Module was comprised of thirty-two cells capable of holding two inmates each and was
designated as a protective custody unit, with inmates who were either cooperators with law enforcement or
charged with sex-related offenses.
Supervisor Monreal indicated that she responded to the Module at approximately 1120 hours. The
assistance was being requested by Correctional Officer Gonzalez for the inmate being housed in Cell 17.
Correctional Officers Gonzalez and R. Zavala #2450 were in the cell attempting to render medical aid to the
inmate in Cell 17.
The inmate in Cell 17 was identified as Darryle Marcell Samuel (DOB Samuel had been in
custody at the Santa Ana Jail since May 24th, 2021. Samuel was on the floor of the cell adjacent to the
toilet. Correctional Officers Gonzalez and Zavala rendered aid to the inmate but he was unresponsive. At
approximately 1130 hours, Orange County Fire Authority (OCFA) personnel from Station 76 arrived on
scene and assumed medical aid of Samuel. At approximately 1141 hours, Emergency Medical Technicians
and Paramedics arrived on scene to assist in rendering aid to Samuel. At approximately 1150 hours, OCFA
Fire Captain Robert Richardson #5760 informed Supervisor Monreal that Samuel was deceased and would
not be transported from the scene.
Supervisor Monreal indicated that at approximately 1152 hours the doors to Cell 17 were closed and the cell

Supervisor Monreal advised us that inmate Samuel came out of his cell to the day room of the Module this
morning at approximately 0714 hours. Samuel watched television and socialized with other inmates and
returned to his cell at approximately 0914 hours. At approximately 0930 hours, Correctional Officer S.
Tolone #3687 conducted a welfare check on Samuel¶s cell and documented the check in a log. At
approximately 1119 hours, two inmate workers named ³ ´and ³ ´were going from cell to cell
cleaning and providing deliveries and then alerted custody staff that there was an inmate hanging in the cell.

interviews of inmates within the Module with Santa Ana Police Department Detective Corporal D. Padron

1435 hours:
Santa Ana Jail Correctional Officer D. Godinez #3694 was assigned to complete a crime scene log. I
checked into the crime scene with Correctional Officer Godinez.
1450 hours:
Investigator Gonzalez, Corporal Padron, and I conducted interviews in the outdoors exercise room of the

I responded to the Santa Ana Police Department and was provided a preliminary briefing of the incident by Sergeant

Sergeant Sarabia informed us that today, at approximately 1125 hours, Santa Ana Jail Supervisor M. Monreal #3181
contacted the Santa Ana Police Department Watch Commander to report that an inmate was down, as a result of a

During her briefing, Supervisor Monreal advised us of the following:
On June 25th, 2021, at approximately 1119 hours, Santa Ana Jail Correctional Officer E. Gonzalez #3366, transmitted
over the radio that he needed medics and additional correctional staff to respond to Module 4C.
As additional medical and correctional staff were responding, Correctional Officers Gonzalez and R. Zavala #2450
were in inside of Cell 17, attempting to render medical aid to the inmate prior to the arrival of medical staff.
The inmate in Cell 17 was identified as Darryle Marcell Samuel. Correctional Officers Gonzalez and Zavala rendered
DLG WR 6DPXHO EXW KH ZDV XQUHVSRQVLYH $W DSSUR[LPDWHO\ KRXUV PHGLFV IURP WKH 2UDQJH &RXQW\ )LUH $XWKRULW\
Station 76, arrived on scene and informed Supervisor Monreal that Samuel was deceased and would not be
transported from the scene.

I was notified by Detective Sergeant M. Sarabia #2565 of an in-custody death that had occurred at the
Santa Ana Jail (SAJ), located at 62 Civic Center Plaza, Santa Ana, CA 92701. At the time of his notification,
I was at my workstation at the Santa Ana Police Department (SAPD).
Additional members of the Crimes Against Persons (CAP) Unit subsequently responded. I began
conducting a background on the decedent, identified as Samuel, Darryle Marcell (DOB
1345 hours:
Detective Sergeant M. Sarabia provided additional information. Present at this time were members of the
CAP Unit and Orange County District Attorneys (OCDA¶s) Investigators. Detective Sergeant M. Sarabia
stated Samuel was found in his single-man cell at approximately 1154 hours. Samuel appeared to have
hanged himself and was subsequently pronounced deceased.
1355 hours:
Members of the CAP Unit and OCDA¶s Investigators walk to the SAJ to attend the briefing. We check in with
CO. D. Godinez #3694
1415 hours:
The briefing was conducted by Correctional Supervisor M. Monreal #3181. The briefing took place in the
multipurpose room of module C4. During the briefing, CS Monreal provided a summary of the initial incident
at the details known at the time.
After the briefing Det. Cpl. P. McClaskey #3110 and I were assigned to interview the inmates in module C4.
Module C4 consists of 32 one and two-man cells. Our conversations with the inmates took place in the
recreational area of the module.
The following is a summation of the interviews.

OCDA Investigators, homicide detectives and members of the Santa Ana Police Department Internal Affairs
Division attended a briefing at the Santa Ana Jail held by Jail Supervisor Monreal. Monreal informed us that
the decedent was housed in cell C#17, located on the top tier. He was in protective custody and housed
alone in a cell. The decedent was identified as Darryle Marcell Samuel, a United States Marshall¶s inmate
housed at the Santa Ana Jail since May 24, 2021. Monreal provided us additional details of the incident
known to her at the time. For details, please refer to Detective M. Gibbons¶ Crime Scene Report.

Sergeant Sarabia provided a briefing to SAPD Homicide Detectives as to the known circumstances related
to the incident.
1355 Hours
I arrived at the Santa Ana Jail booking area located on the first floor. I was advised that the incident
occurred in module 4C cell 17

We were escorted to the multi-purpose room in Module 4C.
1415 Hours
Santa Ana Jail Shift Supervisor M. Monreal #3181 provided a briefing regarding the in custody death and
the status of the initial investigation by correctional officers. The decedent was tentatively identified as
Darryle Marcell Samuel. Samuel was an inmate at the Santa Ana City Jail under booking number
P00221869. Samuel was assigned to cell 17. Although cell 17 is a two-person cell, Samuel did not have a
cellmate. Present were Detectives from the Santa Ana Police Department Homicide Unit, Santa Ana Police
Department Professional Standard Inspectors, Orange County District Attorney Investigators and the
Orange County Crime Lab. I was later paired up with OCDA Investigator C. Brower and assigned to handle
the investigation.
I checked in with Correctional Officer D. Godinez #3694, who was maintaining the crime scene entry/exit
personnel log.
At the conclusion of the briefing, I walked to cell 17 and looked into the cell.
Investigator Brower and I interviewed medical personnel at the Santa Ana Jail. Detective C. Shynn
shadowed during the interviews. It should be noted that Investigator Brower audio recorded all the
interviews. I later received a copy of said interviews, which I booked into SAPD Evidence. The following is
a synopsis of each interview with the medical staff.
1457 Hours
We interviewed Melissa Goyne. Goyne is the interim health services administrator for Naph Care and is a
contract employee for the Santa Ana City Jail. Goyne has been a registered nurse since 2014. Goyne has
many duties in her position. Her duties consist of being a charge nurse, nurse manager, and at times
performs LVN duties. Goyne had 4 medical staff personnel working at the Santa Ana Jail on this day.
Goyne said at approximately 1118 hours, she was in booking (1st floor) when she heard a radio
transmission of a man down in Module 4C (Charlie) over the jail radio. Goyne said that she, along with
Registered Nurses Nicola Orozco and Kristen Sanchez responded to Module 4C. When she arrived, she
saw that LVN¶s Ariadne Zamora and Carolina Pena were already on scene performing CPR on Darryle
Samuel.
Goyne saw Samuel laying on the floor of cell 17 in a supine position. The only injuries she was able to see
on Samuel were some red marks on the front of his neck. She saw a ligature tied around a shelf in the cell.
It looked like a white sheet that had been knotted around the shelf. Goyne made the observation as she
stood outside the cell. She did not enter the cell because CPR was being performed on Samuel. She
observed that Samuel¶s nail beds were white, his fingers and lips were blue in color. Goyne added that
during the life saving measures performed on Samuel, jail medical staff administered Narcan twice with
negative results and started an IV on Samuel. EMS arrived shortly thereafter and they continued to perform
life saving measures on Samuel.
Goyne added that an AED was used on Samuel and the medical strip was in her possession in an
downstairs office.
We concluded the interview.
1514 Hours
Goyne provided me with the AED medical strip, which was booked into SAPD Evidence. A copy of the
medical strip is attached.
1515 Hours
We interviewed Kristen Sanchez. Sanchez is a Registered Nurse and works for Naph Care. She is
assigned to the Santa Ana Jail. Her duties for today was to perform as the charge nurse and work out of

the booking area located on the first floor.
Sanchez stated she was in the booking intake area when she heard a radio transmission (10-49 4 Charlie).
As soon as she heard that call, she grabbed the emergency crash cart and proceeded to 4 Charlie with
other medical staff. As she was on her way, she heard another radio transmission advising 10-49 4 Charlie
man down. When she arrived at 4 Charlie cell 17, other medical staff were on scene. LVN Carolina Pena
and LVN Arianda Zamora were performing CPR on Samuel.
Sanchez saw that Samuel was in a supine position as Zamora and Pena were performing CPR. Samuel
was unresponsive. Samuel¶s limbs and fingertips were blue in color. Samuel was cool to the touch.
Sanchez said that an AED was applied.
Sanchez said that she observed red ligature mark on the front of Samuel¶s neck. She did not observed any
other injuries on his body.
Sanchez said she observed a bed sheet tied to a metal table inside the cell, which was approximately 5 feet
to 6 feet off the ground.
We concluded the interview.
1525 Hours
We then interviewed Ariadne Zamora. Zamora is an LVN and works for Naph Care. She has been
assigned to the Santa Ana Jail for approximately 6 years.
On today¶s date, she was assigned to the 3rd floor of the jail when she heard ³medical,´over the
radio. Zamora said she was the first to respond and say, ³Medical enroute.´Zamora said that the distress
on the officer¶s voice made her think that medical was indeed needed. Zamora grabbed their red medical
bag. Caroline Pena, Nurse Practitioner Rhea Marana and Zamora proceeded to 4 Charlie module. Pena
was ahead of her. As she was responding, she heard over the jail radio that they were needed at cell 17.
Upon her arrival, she observed Correction Officer Zavala and LVN Pena in the cell.
Zamora observed Samuel in a supine position. Pena was attempting to check for a pulse on Samuel. Pena
began to perform CPR on Samuel. Zamora alternated with Pena in performing CPR. An AED was placed
on Samuel. The AED device advised not to shock Samuel and they continued to perform CPR. Zamora
observed that Samuel was pale and his lips were bluish in color.
Zamora said she scanned the cell and observed a sheet hanging, which was tied to a top shelf/table.
Zamora stated Samuel looked familiar to her and did not know him. She observed a ligature mark on his
neck and did not observed any other injuries.
We concluded the interview.
1536 Hours
We interviewed Nurse Practitioner Rhea Marana. Marana works for Naph Care. She has been assigned to
the Santa Ana Jail for approximately 3 years.
On today¶s date, Marana was on the 3rd floor when she heard the LVN¶s (Zamora and Pena) say, ³Let¶s
go!´Marana was advised they were needed in 4 Charlie. When she arrived at 4 Charlie, Marana observed
Samuel in a supine position on the floor and Pena had begun to preform CPR. Marana observed Zamora,
Pena and Correction Officer Zavala performed CPR on Samuel.
Marana did not observe any injuries on Samuel. At first, she believed Samuel might have overdosed.
Narcan was administered to Samuel. An AED was used on Samuel. The AED advised not to shock
Samuel. They continued to perform CPR on Samuel until paramedics arrived.
Paramedics arrived and continued to perform life saving measures on Samuel. The paramedics were in
communication with Dr. Chen from Orange County Global Medical Center. At approximately 1147 hours,
Samuel was pronounced deceased.

1546 Hours
We interviewed Registered Nurse Nicola Orozco. Orozco works for Naph Care. She has been assigned to
the Santa Ana Jail for approximately 8 months.
On today¶s date, she was assigned as the psych nurse and works out of intake, which is located on the first
floor of the jail. At around 1115 hours, she heard a broadcast over the jail radio. The broadcast asked all
officers to respond to 4 Charlie module. They also asked for medical personnel to respond to 4 Charlie as
well. Orozco responded from the 1st floor along with Registered Nurse Sanchez and Nurse Manager
Goyne. Orozco said that she grabbed the crash cart, which contains medical paraphernalia.
She arrived to 4 Charlie and was directed to cell 17. She observed medical staff entering and Correction
Officers standing outside the cell. LVN Zamora had requested the AED, which was placed on Samuel. At
the same time, the medical staff was performing CPR on Samuel. The AED indicated not to shock
Samuel.
Orozco stated that she observed Samuel to have bluish lips. Samuel was motionless and lifeless. Samuel
was not taking in oxygen on his own and had no pulse. Orozco saw a ligature mark below Samuel¶s jawline
(below ears) and did not see any other injuries.
Paramedics arrived and took over life saving measures on Samuel. Samuel was later pronounced
deceased.
Orozco stated that a psych nurse has seen Samuel in the past. Samuel has been seen for depression,
PTSD and Samuel has attempted suicide in the past. She did not know when Samuel last attempted to
commit suicide.
We concluded the interview.
1556 Hours
We interviewed LVN Caroline Pena. Pena works for Naph Care. She has been assigned to the Santa Ana
Jail for approximately 1 year.
On today¶s date, Pena was performing her normal duties. She was assigned to the 4th floor this week. At
around 1115 hours, she was in the medicine room located on the 3rd floor when she heard a ³´
(proceed to the location) call. Pena said that the officer who put out the 10-49 radio traffic sounded
distressed. She then heard over the radio a, ³call to all medical and officers to 4 Charlie.´
Pena stated that she along with Nurse Practitioner Rhea Marana and LVN Zamora responded to 4 Charlie.
Pena said they took a medical bag with them. When she arrived to 4 Charlie, she was directed to cell 17 by
one of the officers. As she arrived to cell 17, Corrections Officer Zavala and Villa were in the cell with
Samuel.
Samuel was on the floor in a supine position. Pena asked to enter and was granted permission. Pena
checked for a pulse on Samuel and did not find one. Samuel felt slightly warm to the touch and his upper
lip was slightly blue. His eyes were partially open. Samuel did not have signs of life. She then began to
perform CPR on Samuel.
Pena said she saw some faint redness around the front part of Samuel¶s neck and did not see any other
visible injuries on Samuel.
Pena said that she did not see anything around Samuel because she was focused on Samuel.
LVN Zamora attached the AED on Samuel. The AED advised not to shock Samuel. Pena continued CPR.
The AED device advised about 4 times not to shock Samuel.

Paramedics/ EMTs arrived and took over life saving measures on Samuel.
Samuel was pronounced deceased at 1147 hours by a doctor at Orange County Global Medical Center.
We concluded the interview.
Investigator Brower and I interviewed inmates housed in 4 Charlie module at the Santa Ana Jail. Detective
C. Shynn shadowed during the interviews. It should be noted that Investigator Brower audio recorded all
the interviews. I later received a copy of said interviews, which I booked into SAPD Evidence. The
following is a synopsis of each interview with the inmates.
1607 Hours
We interviewed Federal inmate in an interview room located on the 1st floor of the jail. The
interview was in Spanish.
is housed in 4 Charlie cell 23 and has been in 4 Charlie for about 2 years. stated that he was
in his cell as his cellmate was cleaning within module 4 Charlie. He heard yelling for
officers to respond. exited his cell and walked over to where was standing. He looked into the
cell and saw a male hanging in cell 17.
began to yell at the officer in charge to respond. Once the officers arrived, said he returned to
this cell. It appeared to him that Samuel used a white sheet to hang himself.
said that he always saw Samuel around in the module but he never talked to him. said that
around 0900 hours this morning, Samuel was on the phone when Samuel became angry. Samuel was
yelling at someone he was talking to on the phone. He believed that Samuel might have been talking to his
wife. At around 0915 hours, Samuel entered his cell. had seen Samuel yelling at someone on the
phone for the last couple of days. Samuel used the payphones on the 2nd floor of the module.

1625 Hours
We then interviewed Santa Ana Jail Federal inmate in an interview room located on
the 1st floor of the jail. is housed in 4 Charlie cell 23 and has been housed at the jail for about 3
years. said that he is one of the workers within the module. His duties include setting up the food
cart, breaking it down and cleaning the module after the food service.
stated that he found Samuel in his cell this morning. He knows Samuel as ³KD.´He has talked to
Samuel before. He did not know if Samuel had any problems with anyone.
This morning he noticed that Samuel was on the phone and was yelling at whomever he was talking to.
Samuel was using one of the phones on the 2nd floor. Samuel is always on the phone. He is normally loud
when on the phone. After the phone call, everyone was locked down around 0915 hours.
said that between 1045 and 1100 hours, his group, which included Samuel, were popped out of their
cells. Meaning that their cell doors were unlocked for lunch. set up the meals for the other inmates.
Everyone is responsible to get their own trays on the 1st floor of the module.
said that after lunch and after everyone is locked down in their cells, he begins to clean the module.
He starts on the outside of cell 17 and begins to wipe down door handles. When he is at cell 17, he called
out to Samuel as he normally does. He wiped the handle, looked inside cell 17 and saw Samuel hanging.
He saw a white sheet around his neck. The sheet was knotted behind his neck. said he pulled the
door to cell 17 and the door opened. Samuel was elevated off the floor and his feet were in front of him.
He saw that Samuel's skin tone was a different color.
said he yelled out to Corrections Officer Gonzalez. was then escorted out of the module.
said that he was not aware of any problems that Samuel was having.

that he wishes that she dies in a car accident. He also told that if he kills himself, he
would haunt her for the rest of his life.
At 0818 hours, Samuel called It appears that Samuel is crying over the phone. They continued to
argue over ¶s unfaithful behavior. Samuel did not want to visit him in jail ever again. He
did not want anything to do with again. Samuel told that he had tried to kill himself twice
on previous occasions. At around 16:55 minutes, into the phone call, Samuel told that he was
going to go kill himself now. He said that he could not take her having someone else¶s baby. Samuel said
that he did not deserve to be no this earth. He told that she drained all the life out of him. Samuel
also told that he did not want to go to medical because he did not care if he died.
Samuel told that he was going to prove to her that he was going to kill himself because it appeared
to him that thought that he was not taking him serious. At the same time, it appeared that
was praying and telling Samuel not to do it.
At 0851 hours, Samuel called He told to get rid of the fetus and demanded to know who
got her pregnant. They continued to argue. Samuel told that if he ever sees her again he is going
to knock her teeth out. Samuel told her to erase all memory of their relationship. Toward the end of the
call, Samuel told that today was his last day on this earth. Samuel said that he was done. Samuel
said he could not keep going through this in life. He told that she fucked him over. Samuel told
not to go visit him because this was the last time he was talking to her.
also told Samuel that she was done with life as well. Samuel told her to go kill herself because she
was not doing anything with her life anyways.

call log (attached) showed that Samuel dialed all 4 times.
June 28, 2021
I viewed surveillance footage from module 4 Charlie. There are 10 cameras within the module. Camera
SAJ122-4C TV Area is aimed toward cell 17 and the phones on the second floor. I viewed footage for June
25, 2021 between 0700 and 1130 hours.
At approximately 0714 hours, Samuel exited his cell and walked to the phones on the 2nd floor. He then
walked downstairs and grabbed a meal. He placed the meal in his cell and returned to the phone on the
2nd floor.
At 0804 hours, Samuel hung up the phone and walked to his cell.
At 0813 hours, Samuel walked out of his cell and goes to the same phone. Samuel is seen going in and out
of his cell. He walked downstairs.
At 0914 hours, Samuel walked into his cell and closed the cell door. It appears that all the inmates walk
back into their cells.
At 1059 hours, lunch was being set up downstairs and inmates began to exit their cells. Samuel¶s cell door
does not open.
At 1111 hours, Inmates began to enter their cells.
At 1113 hours, was seen picking up trash and cleaning outside cells 17, 18 and 19.
At 1118 hours, looked into cell 17. pulled on the cell door and it slightly opened. opened
the door, looked downstairs and began to point into the room. and an unknown inmate appear to be
trying to get jail staffs attention.
Correction Officers run up and enter cell 17. Medical personnel arrive shortly after.
The surveillance footage was later booked into SAPD Evidence.
