Present in the interview was Santa Ana Police Department (SAPD) Investigator JIM GARCIA and SAPD Sergeant MICHELLE MACCHIAROLI. The interview took place in the Orange County Global Medical Center (OCGMC) Emergency Room (ER) in room 13. The following is a summary of the interview with and is not verbatim or necessarily in order of the conversation, but organized for the purpose of clarity. Refer to the digital audio recording for exact quotes, comments and statements. is currently employed at the OCGMC as a Registered Nurse (RN). He has been an RN since 1995 and has worked at OCGMC for two (2) years and four (4) months. On Wednesday, November 4, 2020, was working as a staff nurse in the OCGMC ER when was brought in by the Orange County Fire Authority (OCFA) for treatment. arrived at approximately 0206 hours and was asystole. OCFA paramedics advised that had been found hanging in his jail cell with bedding tied around his neck. Upon arriving at the jail, paramedics found jail medical staff performing cardiopulmonary resuscitation (CPR) on . Paramedics connected to a heart monitor and determined he was asystole. They continued CPR, placed an I-gel airway, and inserted an intraosseous (IO) in right lower leg. When arrived in the ER, saw there was a bag valve mask (BVM) attached to his oral airway and a Lucas Chest Compression System was attached to his chest. saw was cyanotic and his pupils were fixed and dilated. was connected to a heart monitor and was found to be asystole. At approximately 0207 hours was given one (1) dose of epinephrine. At approximately 0208 hours, was given one (1) ampule of sodium bicarbonate. At approximately 0210 hours, was checked for a pulse and found to be pulseless and asystole. At approximately 0211 hours, was given another dose of epinephrine. At approximately 0212 hours, was checked for a pulse and was found to be pulseless and asystole. At approximately 0213 hours ER Doctor placed an endotracheal tube in SA 20-027 Investigator: B. Sheldon, 339 Approved By: Date of Report: 11/05/20 Date: November 5, 2020 Page 3 of 3 At approximately 0214 hours was found to have pulseless electrical activity (PEA), then went into asystole. An ultrasound revealed had no cardiac movement and his pupils were fixed and dilated. Efforts to resuscitate were unsuccessful and at approximately 0215 hours, Dr. pronounced deceased. did not see any apparent or unusual signs of injuries to . stated during the time was in the ER, he was unresponsive and made no statements. Medical staff was unable to establish IV access, therefore no blood was collected from The interview was concluded at 0656 hours.
