Gonzales #349 Approved By: Date of Report: 12/3/2020 Date: December 3, 2020 Page 1 of 2 CASE NAME: CLASSIFICATION: CUSTODIAL RELATED DEATH INTERVIEW OF: RYAN MCCULLOCH, CAPTAIN/PARAMEDIC ORANGE COUNTY FIRE AUTHORITY DATE AND TIME: NOVEMBER 23, 2020 1415 HOURS LOCATION: TELEPHONIC INTERVIEW On Monday November 23, 2020, at approximately 1415 hours, I conducted an audio digital recorded phone interview of Orange County Fire Authority (OCFA) Captain / Paramedic RYAN MCCULLOCH in reference to the Santa Ana Police Department (SAPD) custodial related death involving . MCCULLOCH referred to the OCFA run log during the interview (incident #20-125239). The following is a summary of the interview: At the time of the incident, MCCULLOCH had been a Captain/Paramedic for approximately eighteen (18) years; his training and medical certifications were current. On the date of the incident, MCCULLOCH was assigned to OCFA Engine 71 with the following firefighters: • SETH ANDRES OCFA, Engineer / Firefighter • ALEX SWANSON OCFA, Firefighter/Paramedic • IVAN STRAIGHT OCFA, Firefighter/Paramedic On November 4, 2020, at 0139 hours, OCFA Engine 71 was dispatched to 1090 West Civic Center Drive, Building 62, Santa Ana (SAPD Jail). Gonzales #349 Approved By: Date of Report: 12/3/2020 Date: December 3, 2020 Page 2 of 2 MCCULLOCH said when they entered the jail he saw deputies and jail medical staff providing cardio pulmonary resuscitation (CPR) to . MCCULLOCH also saw had an automated external defibrillator (AED) applied to his torso. MCCULLOCH said he worked with patient paramedic ALEX SWANSON and documented the condition and treatment of . had no pulse, not breathing, and had no blood pressure. Glasgow coma scale showed three, resembling that of a deceased person. At 0150 hours, was intubated followed by an intraosseous infusion line (I/O). At 0158 hours, was administered 1 mg of epinephrine and a second dose at 0204 hours. MCCULLOCH saw no changes in condition, and he remained asystole. MCCULLOCH said was transported to Orange County Global Medical Center (OCGMC). At 0209 hours, MCCULLOCH arrived at OCGMC; he provided the Trauma Staff with all relevant medical information and released patient care to the medical personnel.
