MELNYK #251 Approved By: Date of Report: 07/13/2018 Date: 07/18/2018 Page 1 of 2 CASE NAME: TAYLOR, DONADONY BLAKE CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) INTERVIEW OF: REGISTERED NURSE ORANGE COUNTY GLOBAL MEDICAL CENTER 1001 NORTH TUSTIN AVENUE, SANTA ANA PRESENT: JUSTO CAPACETE, DETECTIVE SANTA ANA POLICE DEPARTMENT DATE AND TIME: 07/02/2018 0253 HOURS LOCATION: ORANGE COUNTY GLOBAL MEDICAL CENTER On Monday, July 2, 2018, at approximately 0253 hours, Santa Ana Police Department (SAPD) Detective JUSTO CAPACETE and I conducted a digital audio recorded interview of Orange County Global Medical Center (OCGMC) Registered Nurse (RN) in reference to the SAPD officer involved shooting of DONADONY TAYLOR. The following is a summary of the interview that is not verbatim or necessarily in order of the conversation, but organized for the purpose of clarity. Refer to the audio recording for exact quotes, comments and statements. On July 1, 2018, at 2353 hours, RN was on duty as a Trauma Clinician at OCGMC. The Critical Trauma Team consisting of a trauma doctor, an anesthesiologist, a respiratory therapist, a trauma clinician, an emergency room RN, an Emergency Medical Technician, a radiology technician and laboratory technician was activated. Orange County Fire Authority (OCFA) Engine #71 notified them of a patient in the field, later identified as TAYLOR ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE BUREAU OF INVESTIGATION INTERVIEW REPORT SA 18-021 Investigator: D. TAYLOR was in full arrest and Cardio Pulmonary Resuscitation (CPR) was in progress. At 0011 hours, TAYLOR arrived at OCGMC. CPR was continued . were administered through the established by the paramedics. Additional rounds of were administered per the Advanced Cardiac Life Support (ACLS) protocol. Doctor placed a int o TAYLOR’s left chest. They never obtained a pulse and CPR was discontinued. The time of death was pronounced at 0025 hours. had been an RN for 3 ½ years.
