Montez #325 Approved By: Date of Report: 09/24/2018 Date: 10/01/2018 Page 1 of 2 CASE NAME: TAYLOR, DONADONY BLAKE CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) INTERVIEW OF: STEVEN CHAVEZ, FIREFIGHTER / PARAMEDIC ORANGE COUNTY FIRE AUTHORITY FIRE STATION 71 1029 WEST 17TH STREET SANTA ANA, CA 92706 PRESENT: N/A DATE AND TIME: JULY 12, 2018 0742 HOURS LOCATION: PHONE INTERVIEW On Thursday, July 12, 2018, at approximately 0742 hours, I conducted an audio digital recorded phone interview of Orange County Fire Authority (OCFA) Firefighter / Paramedic STEVEN CHAVEZ in reference to the Santa Ana Police Department (SAPD) officer involved shooting (OIS) death involving DONANDONY BLAKE TAYLOR. The following is a summary of the interview: At the time of the incident, CHAVEZ was employed as a firefighter paramedic with OCFA. CHAVEZ had been a firefighter for thirteen (13) years and a paramedic for the last three (3) years. CHAVEZ was assigned to OCFA Fire Station 19 but on this day he was working an overtime shift at Fire Station 71. On the date of the incident, CHAVEZ partnered with the following firefighters:  MICHAEL CHESNER OCFA, Firefighter/Paramedic  BRETT CULP OCFA, Captain  ZACHARY SHIRLEY OCFA, Firefighter/Engineer ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE BUREAU OF INVESTIGATION INTERVIEW REPORT SA 18-021 Investigator: P. Montez #325 Approved By: Date of Report: 09/24/2018 Date: 10/01/2018 Page 2 of 2 On July 1, 2018, at 2338 hours, CHAVEZ along with OCFA Engine 71 personnel were dispatched to a shooting on the second floor of the parking structure located at 1020 West Civic Center Drive, Santa Ana. They arrived at 2341 hours. When they arrived, CHAVEZ observed DONADONY BLAKE TAYLOR handcuffed with his hands behind his back and he was lying on the slab lying on his back. Police officers on scene were performing cardio pulmonary resuscitation (CPR). TAYLOR did not have a pulse, was apneic and did not respond to verbal painful stimuli. TAYLOR had a chest seal on mid sternum covering a bullet hole, a bullet hole on the left bicep, and two Taser barbs imbedded on the front of his chest. CHAVEZ had the officers stop CPR and after assessing he still had no pulse and was not breathing. CHAVEZ continued CPR, placed TAYLOR on a Zoll monitor and ventilated TAYLOR with oxygen. Th e Traumatic Cardiac Arrest Protocol was then followed which consisted of: establishing an airway, a rhythm by placing him on a monitor, an intravenous line by doing an intraosseous infusion on the left leg. Due to TAYLOR having Pulseless Electrical Activity (PEA) no shock was advised. They applied a tourniquet to the left bicep to stop the bleeding. They administered a total of four (4) dosages of epinephrine. Due to TAYLOR having diminished air on his right lung they were directed by the base hospital t o perform a needle decompression. At 2358 hours, TAYLOR was placed in a CARE Ambulance and transported to Orange County Global Medical Center – Santa Ana (OCGMC). At 0009 hours, they arrived at OCGMC and transferred TAYLOR’S care to the Emergency Room medical staff. CHAVEZ had no additional information to add. The interview ended at 0756 hours.
