Kendrick #257 Approved By: Date of Report: 07-10-18 Date: 07/11/2018 Page 1 of 9 CASE NAME: TAYLOR, DONADONY, BLAKE CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) TYPE: INITIAL RESPONSE/ SCENE AGENCY: SANTA ANA POLICE DEPARTMENT On Monday, July 2, 2018 , at approximately 0012 hours, Orange County District Attorney (OCDA) Supervising Investigator STAN BERRY contacted me regarding an officer involved shooting involving the Santa Ana Police Department (SAPD). BERRY requested that I respond to the parking structure located at 1020 West Civic Center Drive, in the city of Santa Ana, to assist with the investigation. At approximately 0120 hours, I arrived at the incident location and met with investigators from the OCDA and the SAPD, who were also present to assist in the investigation. At approximately 0206 hours, SAPD Sergeant ABEL OROPEZA, #2499, conducted a briefing of the known circumstances surrounding the incident for the investigators involved. Following the briefing, BERRY assigned SAPD Detective MATTHEW MCLEOD, #2770, and I to supervise the processing of the scene by Orange County Crime Lab (OCCL) personnel. Kendrick #257 Approved By: Date of Report: 07-10-18 Date: 07/11/2018 Page 2 of 9 At approximately 0230 hours, OCCL personnel began processing the scene, which included diagraming, photography, and the collection of evidence. SCENE DESCRIPTION The weather was cool, dark, and clear. The incident occurred at approximately 2337 hours, on Sunday, July 1, 2018, during the hours of darkness, on the second floor of The Stadium parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The scene was cordoned off by yellow and red police line tape. Uniformed SAPD personnel were stationed to prevent access to, and preserve evidence within the scene. At 2350 hours, SAPD Officer ALEX GOMEZ, #3568, began maintaining a crime scene log for incoming and outgoing personnel. The parking structure and surrounding area at 1020 west Civic Center Drive, in the city of Santa Ana, is described as follows: The Stadium Parking Structure, situated at 1020 West Civic Center Drive, in the city of Santa Ana, is a four story parking structure, including parking on the roof. The 600 block of Coach Dick Hill Way is situated on the east side of the parking structure and runs in a north/south direction. The 600 block of North Boyd Way is situated on the west side of the parking structure and runs in a north/south direction. There are entrances to the parking structure from Boyd Way and from Coach Dick Hill Way. There is an elevator and stairwell allowing access to all floors on the northeast and southeast portions of the parking structure. There were six SAPD police vehicles and nine civilian vehicles within the crime scene perimeter. Those vehicles are described as follows: VEHICLES IN SCENE  Vehicle #1 –2011 Ford Crown Victoria Police Interceptor, four door, black and white, SAPD Vehicle Number 865/59925, vehicle license number 1415405. This vehicle was assigned to SAPD Officer GAVIN ROELOFS, #3514, who was the sole occupant. This vehicle was S.A. 18-021 Investigator: M. Kendrick #257 Approved By: Date of Report: 07-10-18 Date: 07/11/2018 Page 3 of 9 parked facing east, on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was running, unlocked, with the keys in the ignition, and the trunk open. The vehicle's passenger spot light was on and facing forward. There was a 12-gauge shotgun (serial #RS83442Z) locked in the rifle rack between the driver and passenger seat. There was one cartridge in the sidesaddle carrier and four cartridges in the chamber. There was a less than lethal Defense Federal Technologies (DFT) 40 mm Launcher (serial #D33255) inside a hard plastic case in the trunk. The launcher was equipped with EOTech optics and nylon sling. There were three less than lethal cartridges in a nylon carrier attached to the buttstock and five cartridges loose in the black case. This vehicle was on scene at the time of the shooting incident.  Vehicle #2 –2011 Ford Crown Victoria Police Interceptor, four door, black and white, SAPD Vehicle Number 818/59941, vehicle license number 1415385. This vehicle was assigned to SAPD Officer IVAN VERA, #3325, who was the sole occupant. This vehicle was parked facing east, on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was running, locked, with the keys in the ignition. The vehicle's overhead rear red and blue emergency lights were activated. There was a Daniel Defense DDM4 carbine rifle (serial #DDM4057155) locked in the rifle rack between the driver and passenger seat. There was a 30 round capacity magazine in the magazine well. The rifle was equipped with an Aim point optic, attached light mount, and nylon sling. There was a less than lethal Lewis Machine & Tool (LMT) 40 mm Launcher (serial #FW12554) inside a hard plastic case in the trunk. The launcher was equipped with aim point optics and nylon sling. This vehicle was not on scene at the time of the shooting incident.  Vehicle #3 –2011 Ford Crown Victoria Police Interceptor, four door, black and white, SAPD Vehicle Number 864/59905, vehicle license number 1408785. This vehicle was assigned to SAPD Officer PATRICK MARSHALL, #3448, who was the driver, and San Bernardino County Sheriff Deputy ERIC MASTEL, #G9453, who was the passenger. This vehicle was parked facing east, on the second floor of the parking structure, located at 1020 West Civic Center Drive in the city of Santa Ana. There was a 12-gauge shotgun (serial # RS846922Z) locked in the rifle rack between the driver and passenger seat. There was four cartridges in the sidesaddle carrier and four cartridges in the chamber. There was a Daniel Defense DDM4 carbine rifle (serial #DDM4057333) locked in the rifle rack between the driver and passenger seat. There was a 30 round capacity magazine in the magazine well. The rifle was equipped with an Aim point optic, attached light mount, and nylon sling. This vehicle was on scene at the time of the shooting incident.  Vehicle #4 –2005 Chevy Tahoe, four door, black and white, SAPD Vehicle Number 821/29507, vehicle license number 1134985. This vehicle was assigned to SAPD Officer Sergeant ABEL OROPEZA, #2499, who was the sole occupant. This vehicle was parked facing west, on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was running, unlocked, with the keys in the ignition, and the driver door open. There was a Colt AR15 Carbine (serial #1776361) loose in the rear back seat of the Tahoe. There was a 30 round capacity magazine in the magazine well. The rifle was equipped with light mount, optics, and nylon sling. This vehicle was not on scene at the time of the shooting incident.  Vehicle #5 –2017 Ford Explorer, four door, black and white, SAPD Vehicle Number 831/60090, vehicle license number 1510501. This vehicle was assigned to SAPD Officer MICHAEL GRIFFITH, #3422, who was the sole occupant. This vehicle was parked facing west, on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was running, unlocked, with the keys in the ignition, and the driver door open. There was a Daniel Defense DDM4 carbine rifle (serial #DDM4057317) locked in the rifle rack between the driver and passenger seat. There was a 30 round capacity magazine in the magazine well. The rifle was equipped with an Aim point optic, attached light mount, and nylon sling. This vehicle was not on scene at the time of the shooting incident.  Vehicle #6 –2005 Chevy Tahoe, four door, black and white, SAPD Vehicle Number 841/29508, vehicle license number 1134984. This vehicle was assigned to SAPD Officer Sergeant GARY COUSO, #2867, who was the sole occupant. This vehicle was parked facing S.A. 18-021 Investigator: M. The vehicle was running, unlocked, with the keys in the ignition. There was a Sig Sauer MCX Carbine (serial # 63C029065) inside a soft nylon case in the rear back seat of the Tahoe. This vehicle was not on scene at the time of the shooting incident.  Vehicle #7 – 2007 Chevy Astro van, four door, white, California license plate . The registered owner was Anaheim, CA 92801. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #8 – 2014 Ford Fusion sedan, four door, silver California license plate . The registered owner was Costa Mesa, CA 92627. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #9 – 2018 Ford F-150 P/U, four door, white, VIN# . The registered owner was . This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #10 – 2017 Honda Civic sedan, two door, white, California license plate . The registered owner was , Apartment , Fullerton, CA 92832. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #12 – 2012 Honda Civic sedan, four door, gray, California license plate . The registered owner was , Anaheim CA 92805. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #13 – 2006 Ford F-150 P/U, four door, black, California license plate . The registered owner was Jurupa Valley, CA 91752. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #14 – 2016 Hyundai Elantra sedan, four door, red, California license plate . The registered owner was Santa Ana, CA 92704. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. The vehicle was locked and unoccupied. This vehicle was on scene at the time of the shooting incident.  Vehicle #15 – 2017 Ford Fusion sedan, four door, silver, California license plate . Record not on file. This vehicle was parked facing south, in a marked painted stall on the second floor of the parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana. Kendrick #257 Approved By: Date of Report: 07-10-18 Date: 07/11/2018 Page 7 of 9 SCENE EXAMINATION There was a long metal pipe (Evidence Marker #1) leaning upright against the south wall, on the second floor of The Stadium parking structure, located at 1020 West Civic Center Drive, in the city of Santa Ana, and south of Vehicle #10. There was a long wire (Evidence Marker #2) leaning upright against the south wall on the second floor of The Stadium parking structure, and west of Evidence Marker #1. There were two metal bars (Evidence Marker #3) on the ground of the second floor of The Stadium parking structure, and south of Vehicle #1. There was one Taser probe, and one Taser probe with attached wire, one partial gray Taser door, and several AFID (anti felon identification) tags (Evidence #4), on the ground, of the second floor of The Stadium parking structure, and southwest of Vehicle #1. There was a Taser with attached deployed cartridge and wire, a partial gray Taser door (Evidence Marker #5) on the ground, of the second floor of The Stadium parking structure, and to the left rear wheel of Vehicle #1. There was a cartridge case (Evidence Marker #6), headstamp WIN 40 S&W, on the ground of the second floor of The Stadium parking structure, and east of Vehicle #2. There was a cartridge case (Evidence Marker #7), headstamp WIN 40 S&W, on the ground of the second floor of The Stadium parking structure, and east of Vehicle #2. EVIDENCE COLLECTION OCCL Forensic Scientist HUBER collected the following items of evidence. At approximately 0415 hours, I conducted a final sweep of the perimeter of the scene in an attempt to locate any additional evidence. No additional evidence was located. At approximately 0420 hours, I turned control of the scene over to SAPD Officer GOMEZ, #3568. S.A. 18-021 Investigator: M.
