TYPE: INITIAL RESPONSE – HOSPITAL SCENE

On Monday, July 2, 2018, at approximately 0009 hours, Orange County District Attorney
(OCDA) Supervising Investigator STAN BERRY contacted me regarding an officer involved
shooting incident involving the Santa Ana Police Department (SAPD). BERRY requested I respond
to the SAPD Station assist with the investigation.
At approximately 0121 hours, I arrived at 1020 West Civic Center Drive and met with
investigators from the OCDA, SAPD, Orange County Crime Lab (OCCL) and Orange County
Coroner’s Office (OCCO) who were present to assist in the investigation.
At 0206 hours, SAPD Sergeant ABEL OROPEZA #2499 conducted a briefing of the known
circumstances surrounding the incident for the personnel involved in the investigation. After the
briefing, BERRY requested SAPD Detective GUSTO CAPACETE and I conduct the hospital scene
investigation and witness interviews at the Orange County Global Medical Center (OCGMC), 1001
North Tustin Avenue, Santa Ana. CAPACETE and I conducted the following audio digital recorded
interviews:
Care Ambulance, Emergency Medical Technician
Registered Nurse, OCGMC
Trauma Surgeon, OCGMC

supplemental interview reports and digital audio recordings for each subject.
The scene was located inside Trauma Room 1 at OCGMC. Trauma Room 1 was identified
by a placard outside the door which indicated “T-1.” The room was being guarded by SAPD
Officer BLAKE KELLY #3378.
Orange County Crime Lab Forensic Specialist LENORA PENA-SANCHEZ arrived to
document the hospital scene. PENA-SANCHEZ took nineteen (19) digital photographs of the
scene under FR# 18-49132. Coroner Investigator PAUL HOAG processed TAYLOR’s body under
OCCL Case #18-03011-HO.
The body of DONADONY TAYLOR was lying supine on a bed in the middle of the room with
his head towards the west and his feet towards the east. TAYLOR’s lower torso was covered by
a white hospital sheet and blanket.
During a visual examination of TAYLOR’s body the following items were observed:
Apparent gunshot wound near the sternum.
Apparent gunshot wound to the left upper arm and left arm pit.
(5) electro-cardiogram (EKG) leads and pads on the torso.
White hospital identification bracelet on the right wrist.
Red hospital identification bracelet on the right wrist.
Handcuff attached to the right wrist.
Intravenous (IV) line in the right forearm.
Blood pressure cuff on the right arm.
Intubation tube and harness on the head.
Chest tube in the left chest.
Pulse oximeter lead attached to left ring finger.
Thoracostomy needles in the left and right upper chest.
(2) Automated External Defibrillator (AED) pads on the front torso.

Intraosseous Infusion (IO) line attached to the left shin.
Brown sock on the right foot.
Black short pants cut off under the body.
Grey underwear cut off under the body.
Black rubber glove on the right hand.
HOAG took photographs of TAYLOR during his examination and collected fingerprints
from TAYLOR’s right hand which were documented under OCCO case 18-03011-HO.
from Traditional Funeral Services arrived to transport TAYLOR to the
Orange County Coroner’s Office located at 1071 West Santa Ana Boulevard, in the city of Santa
Ana. TAYLOR was placed into a body bag and a toe tag was placed on left toe. The body bag was
tagged and sealed at 0458 hours.
