<br /> SA-15-005 <br />Investigator: Bruce Linn #327 Approved By: <br />Date of Report: 5/4/2015 Date: 07/13/2015 <br />Page 1 of 4 <br /> <br /> <br />CASE NAME: OSORIO, BENITO <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />TYPE: HOSPITAL INVESTIGATION <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />CORONER CASE #: 15-01178-KI <br />OCCL FR#: 15-43742 <br />On Wednesday, March 11, 2015, at 1345 hours, I was contacted by Orange County <br />District Attorney (OCDA) Investigator DOMINGO CABRERA regarding an officer involved <br />shooting incident involving the Santa Ana Police Department. CABRERA requested I respond to <br />the Special Assignments Unit office for an initial briefing. Once briefed, I was assigned by OCDA <br />Supervising Investigator MARK GUTIERREZ to respond to Western Medical Center, Santa Ana <br />where the suspect had been transported from the scene by paramedics. <br />At 1435 hours, I arrived at Western Medical Center, Santa Ana Emergency Room (ER). I <br />was directed by emergency room security to ER room T1. Santa Ana Police Department Patrol <br />Officer DON KOZAKOWSKI #1888 was present in the room. The deceased suspect was supine <br />on a gurney in Western Medical Center, Santa Ana ER room T1. Room T1 was a standard, large <br />sized ER trauma room with a variety of medical care equipment, for the purpose of emergency <br />and trauma patient care.
