S.A. 15-005 <br />Investigator: ERIC WISEMAN Approved By: <br />Date of Report: 03/17/15 Date: 03/19/2015 <br />Page 1 of 2 <br /> <br /> <br /> <br />CASE NAME: OSIRIO, BENITO <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (FATAL) <br />TYPE: INITIAL RESPONSE <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br />On Wednesday, March 11, 2015, at approximately 1330 hours, Orange County District <br />Attorney (OCDA) Supervising Investigator MARK GUTIERREZ contacted me regarding an officer <br />involved shooting incident involving the Santa Ana Police Department (SAPD). GUTIERREZ <br />requested I respond to the area of South Main Street and Pine Street, in the city of Santa Ana, <br />and assist with the investigation. <br />At 1425 hours, I arrived at the incident location and met with investigators from the <br />OCDA and from the SAPD, who were present to assist in the investigation. <br />At 1513 hours, SAPD Sergeant JEFF SMITH conducted a briefing of the known <br />circumstances surrounding the incident for the personnel involved in the investigation. <br />Following the briefing, GUTIERREZ requested SAPD Investigator RICK DIAZ and I conduct a <br />canvass for potential witnesses to the shooting incident. <br />During the canvass, DIAZ and I interviewed eight individuals who heard the shooting <br />incident.
