The preliminary examination of the exterior of the Toyota pick-up truck revealed the
following:
There was a moderate amount of blood splattered along the exterior side of the left
rear, passenger door and quarter panel. The glass in the driver’s door was shattered and was
missing from the door frame. There was a bullet hole through the driver’s door, in the door
frame area surrounding the driver side window, located above the exterior door handle. This
appeared to be an entrance hole on one side of the door, and an exit hole on the other side,
caused by a bullet traveling through it. The driver side mirror was broken and appeared to have
been struck by a bullet or a fragment of a bullet.
There was a bullet protrusion from the interior of the driver’s door frame, between the
driver’s door and the windshield of the vehicle. This appeared to be an interior bullet strike
when the door was open. There were two bullet holes in the driver’s door the exterior door
handle. There was one bullet hole in the left rear passenger door, near the exterior door
handle of the driver’s door. There appeared to be a bullet strike mark in the center of the left,
rear, passenger door. There were two bullet holes in the exterior, left side (driver’s side) of the
tailgate. There was an exterior bullet hole in the roof of the vehicle; located just behind the
driver’s seat, and appeared to be an exit hole.
The preliminary examination of the interior of the Toyota pick-up truck revealed the
following:
On top of the center counsel, next to the gear shift lever and between the driver and
passenger seat was a “Smith and Wesson” gun case, blue in color which was open. Previously
at the Officer Involved Shooting (OIS) scene on Main Street, I had noted that there was a live,
9mm, ammunition round sitting loose inside the open gun case; however, I now observed that

round was no longer in the case. I then observed a 9mm ammunition round sitting on the
driver’s seat next to a black cellular telephone, later documented in photograph images 195
and 196. OCCL personnel told me the ammunition round was first documented at the OIS
scene as being in the open gun case; however, it had inadvertently shifted during the vehicles
transportation to the OCCL, and had subsequently fallen out of the case onto the driver’s seat.
There was apparent blood stains on the driver’s seat, steering wheel and interior
driver’s door panel. On the right, front, passenger’s seat was a black, back-pack, blue sweat
shirt, and other miscellaneous clothing and personal items. On the right, front, passenger
floorboard, I observed an expended cartridge case and a metal bullet fragment.
Examination of the Toyota pick-up truck revealed the following;
There was an apparent bullet strike into the exterior, driver’s door, mirror (Evidence
Photo Marker A). A bullet hole was located in the exterior side of the driver’s door above, and
to the left of the exterior door handle (Evidence Phot Marker B). It appeared the bullet entered
the vehicle and exited the interior driver’s door panel and terminated into an unknown area
inside the truck (Evidence Photo Marker B-1)
A bullet hole was located in the exterior side of the driver’s door, just below the exterior
door handle (Evidence Photo Marker C). It appeared this bullet exited the interior driver’s door
panel and terminated into an unknown area inside the truck (Evidence Photo Marker C-1).
A bullet entrance and exit hole was located in the window frame of the driver’s door
(Evidence Photo Marker D). It appeared this bullet traveled through the door (Evidence Photo
Marker D-1). The bullets termination point was unknown.
A bullet hole was located on the interior door panel of the driver’s side door next to the
doors locking mechanism (Evidence Photo Marker E). A bullet exit hole was located at the

same location on the interior side of the driver’s door, terminating into the door panel
(Evidence Photo Marker E-1 and E-2).
A bullet hole was located in the left rear door, near the exterior door handle of the
driver’s door (Evidence Photo Marker F). It appeared this bullet entered the vehicle and
traveled through the “A” pillar (Evidence Photo Marker F-1) then through the back sides of the
driver and right front passenger seats, terminating into the “B” pillar, panel cover (Evidence
Photo Marker F-7)
A bullet strike (skip) was located in the middle of the left rear passenger door (Evidence
Photo Marker G). A bullet hole was located in the interior frame between the driver’s door and
the windshield (Evidence Photo Marker H). The bullet terminated within the vehicle frame and
did not exit the exterior side of the frame. The bullet left an exterior protrusion on the outside
of the frame; however, this bullet was ultimately located within the interior frame itself
(Evidence Photo Marker H).
A bullet hole was located in the upper left exterior portion of the trucks tailgate
(Evidence Photo Marker I). The bullet terminated into and was recovered from the interior
panel of the trucks bed (Evidence Photo Marker I-1).
A bullet hole was located in the lower left exterior portion of the trucks tailgate
(Evidence Photo Marker J). The bullet was located inside the interior left wheel well (Evidence
Photo Marker J-1, J-2 and J-3).
There was a bullet hole in the ceiling panel above and to the rear of the driver’s seat
(Evidence Photo Marker K). There was an exit hole at the same location, on the exterior side of
the roof (Evidence Photo Marker K-1). The bullets termination point is unknown.

On the center console was a box, containing 30 cartridges of “Blazer” 9mm
ammunition. Further determined to be; Luger, 9mm, 115 Grain, full metal jacketed (FMJ). The

information affixed to it; “Smith and Wesson,” Model #6906, 9mm, 3 ½ inch barrel and serial
#VYY9444. The bed of the truck contained a pair of brown work boots and miscellaneous trash.
An Orange traffic cone and several personal items, un-related to this investigation, were
located throughout the vehicle.
