On 12/01/24, at approximately 1708 hours, the Santa Ana Police Department Communications Center received a call
of a subject armed with a shotgun. The reporting party indicated he observed the suspect in the area of 300 E. 1st
Street, Santa Ana. Officers responded to the area but were unable to locate the suspect.
At approximately 1732 hours, the Santa Ana Police Department Communications Center received a call of a male
armed with a rifle and loading it. The call came from an employee at 116 N. Broadway.
Santa Ana Police Officers responded to the area. Upon arrival, Santa Ana Police Officers encountered the suspect,
later identified as , south of Casa Linda Furniture, 116 N. Broadway. The encounter lead to an
Officer Involved Shooting.
Officers contacted witnesses in the area and transported them to the Santa Ana Police Department for an interview.
California Department of Justice Special Agent Michael Wyatt requested Detective G. Bodnar #3337 and I conduct
the interview with one of the witnesses. The witness was identified as .
SUNDAY, DECEMBER 1, 2024 – 2242 HOURS:
Detective Bodnar and I interviewed . The interview took place at the Santa Ana Police Department, in one of the
3rd floor interview rooms, interview room #3. The following is a summary of the Spanish video recorded
interview. After introductions, told us the following.
On Sunday, 12/01/24, night, she was working at the taco trailer, Diaz Tacos, with her coworker, . They
began their set up at 1600 hours and were scheduled to work until midnight, 0000 hours. The lunch trailer was parked
adjacent to Guadalajara Furniture, 117 N Broadway. At the time the of the incident, Reyes said there were no
customers at the lunch trailer.
While tending the lunch trailer, her attention was drawn to a police vehicle that traveled north on Broadway. She could

On 12/01/24 at approximately 1735 hours, Corporal J. Rosales and I responded to an officer involved shooting at the
area of 2nd Street and Broadway. I arrived on scene and was tasked with the crime scene log who I later relinquished
to Officer A. Sanudo.
I then was tasked with obtaining statements from possible witnesses that were inside the Guadalajara Furniture Inc.
store located at 117 N Broadway while the incident occurred. I spoke to a to 4 witnesses who stated the following:

NARRATIVE:
Sunday, December 1, 2024 – 1738 hours:
Detective Sergeant M. Lemmon #3174 notified me of an officer-involved shooting that had just occurred in the area of
Broadway and 2nd Street in Santa Ana. Detective Lemmon requested I respond and assist with the investigation.
1841 hours:
I arrived near the scene and staged at the northeast parking lot of 1st Street and Broadway. The crime scene was
identified by crime scene tape on all sides. Uniformed patrol officers and marked police vehicles were present to
secure the scene, preventing vehicular and pedestrian traffic access.
2017 hours:
I checked into the scene with Officer A. Carrillo #3424.
2018 hours:
Sergeant L. Barragan #2937 briefed Detectives from the Santa Ana Police Department Homicide Unit, Inspectors
from the Santa Ana Police Department Professional Standards Unit, and Investigators California Department of
Justice on the initial investigation by patrol officers.
Sergeant Barragan said that on 12/01/24, at approximately 1708 hours, the Santa Ana Police Communications Center
received a phone call from a subject in the area armed with a shotgun. Officers arrived in the area but were unable to
locate anyone matching the description provided by the calling party.
At approximately 1732 hours, an additional caller advised a male was loading a rifle in the area of 116 N. Broadway.
At approximately 1733 hours, Officer I. Ibarra #3178 and Officer L. Casillas # 3755 located a subject, later identified
as , who matched the description previously provided by the calling parties. Officer Casillas
and Officer Ibarra contacted , who was standing along the western sidewalk of Broadway, between 1st
Street and 2nd Street. During the contact with , an Officer Involved Shooting occurred. was
transported to OC Global Medical Center, where he was pronounced deceased by Dr. at 1826 hours.
Santa Ana Police Officers canvassed the area for witnesses and surveillance footage. Refer to the attached
supplemental reports for further details on witness statements and surveillance footage obtained from the area of the
incident.
2043 hours:
Sergeant Barragan concluded the briefing. I walked the exterior of the scene, which was located near the eastern
driveway entrance to 300 W. 2nd Street (Artists Village Apartments) and extended to the southbound traffic lanes of
Broadway. The apartment complex had an open driveway entrance that led to a first-floor parking area. Three
additional floors were above the first-floor parking lot. There were additional apartments with eastern-facing windows
that provided an overhead view of the scene.
I was tasked with canvassing 300 W. 2nd Street and contacting the eastern-facing apartments that looked onto
Broadway with Department of Justice Special Agent M. Beavers, hereafter referred to as SA Beavers.
2142 hours:
SA Beavers and I contacted , who resided in apartment . recalled that sometime after
1700 hours, he was inside his office, which had windows that provided a view of Broadway. heard a loud
commotion coming outside. later heard approximately ten gunshots over three to four seconds.
looked outside his window and saw police cars with their emergency lights and sirens on. Due to the angle
from his window, saw officers directing their attention to the area near the garage entrance. The officers
appeared to be holding firearms at the time. was unable to see who the officers were addressing near the
garage entrance, and he had no further information.
SA Beavers and I spoke with girlfriend, who also resided in apartment # . was in a
separate room and heard approximately five gunshots outside. heard sirens in the area and later looked outside

an apartment window. saw multiple police cars in the area. had no further information.
2153 hours:
SA Beavers and I attempted to contact the residents of apartment but received no answer at the door. I left my
business card at the front door in an attempt to have the residents contact me later.
2154 hours:
SA Beavers and I contacted , who resided in apartment . advised she was not home at
the time of the incident and did not see what occurred. had no further information.
There were no additional apartments on the second floor with eastern-facing windows overlooking Broadway.
2158 hours:
SA Beavers and I contacted , who resided in apartment said that at approximately 1730
hours, he was in his living room when he heard six or seven gunshots over a 1.5-second time span. I asked if
he heard anything prior to the gunshots, and he said he did not.
After hearing the gunshots, looked out of his balcony and saw a police vehicle parked partially inside the
parking garage on the first floor. The emergency lights were activated, and a police officer was standing towards the
rear passenger portion of the vehicle.
After a few seconds, returned to his residence and checked on his younger brother, , who was also
inside at the time of the incident. drove his brother to his mother's residence, located near the intersection of
Broadway and Washington Avenue in Santa Ana. later returned and saw additional police vehicles securing the
scene. had no further information.
2208 hours:
SA Beavers and I attempted to contact the residents of apartment # but received no answer at the door. I left my
business card at the front door in an attempt to have the residents contact me later.
2209 hours:
SA Beavers and I attempted to contact the residents of apartment but received no answer at the door. I left my
business card at the front door in an attempt to have the residents contact me later.
There were no additional apartments on the third floor with eastern-facing windows overlooking Broadway.
2211 hours:
SA Beavers and I contacted who resided in apartment # said that at approximately 1900
hours, he was sitting in his living room when he heard what sounded like two men arguing outside. Elliot could not
understand what was being said between the individuals outside. Shortly after, heard approximately 16 gunshots
coming from outside. believed the gunshots were fired over a 15-second time span. did not look outside or
see any activity occurring outside. had no further information.
SA Beavers and I contacted girlfriend, , who also lived there. said she was also in the
living room with when she heard two males arguing outside. heard one of the individuals shout "fuck
you." Another individual responded, "fuck you back." immediately heard approximately 10 or 12 gunshots,
followed by police sirens responding to the area. remained inside the residence. had no further
information.
2221 hours:
SA Beavers and I contacted at apartment . advised that she lived in apartment #
but was visiting her friend, , who resided in apartment . apartment was on the opposite
side of the hallway and did not have any windows or views onto Broadway. advised that she was inside her
residence when, at approximately 1700 hours, she heard between 10 to 15 gunshots coming from outside.
had no further information.
SA Beavers and I contacted , who advised that she was not home at the time of the incident. left

her residence on 11/30/24 and returned to her residence on 12/01/24 at approximately 2015 hours. advised
that she took photographs of the scene from her balcony. showed me an image on her phone that depicted an
overhead picture of the crime scene. enhanced the image and showed me what appeared to be a black
shotgun with a brown forearm and grip. had no further information.
2234 hours:
SA Beavers and I contacted , who resided in apartment . advised that he called about the
individual with a shotgun in the "staircase" earlier in the day. later described the unknown male as a Hispanic
male, 30 to 40 years old, last seen wearing a brown or green jacket, pants, and a beanie or hood over his head.
believed the male's face was "dirty," and he had "stubble" for facial hair.
On 12/01/24, at approximately 1746 hours, was walking his dogs on Broadway. He entered the complex through
an eastern-facing pedestrian door located next to the driveway entrance on Broadway.
was walking through a hallway towards the stairs located on the northern end of the building. As walked
through the hallway, he passed an open doorway along the southern wall of the hallway. The doorway led to a
stairwell that allowed access to an underground parking lot. recalled an unknown man was walking up the stairs,
towards the open doorway. The unknown man was carrying a black shotgun with a brown "pump." saw the male
carrying the shotgun vertically in his hand. The butt of the shotgun was being held in the palm of the unknown male's
hand, and the barrel was lying along his shoulder. saw the barrel of the shotgun reach the top of the unknown
male's shoulder.
had not previously seen the unknown male in the area or within the apartment complex. was not sure if he
would be able to recognize the unknown male if he saw him again.
walked the stairs to his apartment and called the property manager, " ." informed the manager of the
man carrying the shotgun and his location. later looked outside through a window that faced Broadway and saw
several police cars on Broadway. I asked if he had heard gunshots, and he said he had not. had no further
information.
After speaking with , SA Beaver and I walked through the downstairs parking lot and found no evidence related to
. I saw multiple surveillance cameras affixed to the ceiling. I contacted Security Guard ,
who advised that the leasing office had access to view the footage captured by the surveillance cameras. The leasing
office was closed at the time SA Beavers and I conducted a walkthrough.
2313 hours:
I checked out of the scene with the scribe, Officer Carrillo. Detective F. Gomez #2706 was designated as the case
agent for this incident on behalf of the Santa Ana Police Department.

Sunday, December 1, 2024, 1745 hours:
Sergeant M. Lemmon #3174 of the Santa Ana Police Department Homicide Unit called to advise me that an Officer
Involved Shooting had just occurred and requested I respond to assist with the investigation. Sergeant Lemmon told
me to respond to the area of west 1st Street and Broadway in the City of Santa Ana, County of Orange.
1910 hours:
I arrived at the location and waited for additional personnel to arrive.
2017 hours:
I checked in with Officer A. Carrillo #3424, who was the scribe at the scene.
2018 hours:
Sergeant L. Barragan #2937 of the Santa Ana Police Department Patrol Division provided briefing to Special Agents
of the State of California Department of Justice, and Homicide Detectives as to the circumstances surrounding the
case.
Sergeant L. Barragan stated that at 1708 hours, the Santa Ana Police Department Communications Center received a
call stating a male Hispanic, between the ages of 45-50 years old, 5-06 to 5-07, 180 – 190 lbs., with long hair and a
brown shotgun was seen walking eastbound on 1st street from Spurgeon. Officer M. Coleman #3767 arrived on
scene and he was unable to locate the suspect. Officer Coleman requested responding units do an area check for the
suspect.
1733 hours:
Santa Ana Police Department Patrol Unit 823A occupied by Officer’s I. Ibarra #3178 and L. Casillas #3755 located the
suspect in the area of 2nd Street and Broadway at which time an officer involved shooting occurred.

Orange County Fire Authority Engine #75 arrived at the scene and transported the suspect to Orange County Global
Medical Center where he was pronounced deceased by Dr. at approximately at 1826 hours.

2120 hours:
At this time, I checked out of the crime scene with Officer Carrillo who was the scribe at the scene.
2133 hours:
At this time, I arrived at OC Global Medical Center located at 1001 N. Tustin Ave., in the City of Santa Ana, County of
Orange. I met with Officer Coleman #3767 in the Prompt Care Room – Bed #A, where he was standing by with the
suspect’s body.
2145 hours:
Santa Ana Police Department Crime Scene Investigations Officer Briano #3173 arrived to the hospital.
2150 hours:
Orange County Sheriff’s Department Senior Deputy Coroner A. Tasse #9396 arrived to the hospital.

Senior Deputy Coroner Tasse removed the white sheet that was covering the body of . I observed
be laying nude in a supine position with medical paraphernalia and a chest tube on the right side of his
body. I observed to have a laceration to the left side of his chest, caused by thoracotomy procedure.
I observed the following gunshot wounds on Rodriguez body:
Gunshot wound to the right side of the face.
Gunshot wound to the left side of the face.
Gunshot wound to the neck.
Gunshot wound to the upper left chest.
Gunshot wound to left bicep.
Gunshot wound to the left inner forearm.

Senior Deputy Coroner A. Tasse place the toe tag on suspects left foot and sealed the bag with Seal #0236409. Prior
to sealing, the bag Senior Deputy Coroner A. Tasse used the Portable Livescans machine in attempt to identify the
suspect. The suspect was identified as .
2243 hours:
from Traditional Funeral Services transported the body to the Orange County Coroner’s Office located
at 1071 W. Santa Ana Blvd., in the City of Santa Ana, County of Orange.
2300 hours:
At this time, we cleared the hospital, and I returned to the Santa Ana Police Department to continue to assist with the
investigation.

shooting in the area of 200 N. Broadway. Sergeant Lemmon requested I respond and assist with the investigation.
1930 hours:
I arrived at the southern perimeter of the crime scene at 1st Street, east of Broadway. Santa Ana Police Department
Officers in marked patrol vehicles were providing security for the crime scene, preventing access into the scene by
unauthorized vehicles and pedestrians.
2017 hours:
I checked into the scene with the designated scene scribe, Officer A. Carrillo #3424
2018 hours:
Santa Ana Police Department Sergeant L. Barragan #2937 conducted a briefing with Detectives from the SAPD
Homicide Section and California Department of Justice Investigators regarding the initial response to the scene and
the preliminary investigation done by patrol personnel.
2041 hours:
Sergeant Barragan concluded the briefing. At the conclusion of the briefing, Detective F. Gomez #2706 was assigned
as the case agent. Detective Gomez assigned me to document the Officer processing.
2120 hours:
I signed out of the scene with Officer Carrillo and responded to the Santa Ana Police Department.
2213 hours:
The Officer processing was conducted in the Internal Affair's office located on the 4th floor of the Santa Ana Police
Department. This is only a summary of the Officer processing.
Department of Justice Special Agent Alvin Hernandez, Department of Justice Senior Criminalist Gina Williams, SAPD
Internal Affair's Nicholas Guijarro #3482, SAPD Officer Luis Casillas #3755, Casillas's representative Attorney
and I were present.
SAPD Officer Casillas is currently assigned as a patrol officer in the field operations division of the Santa Ana Police
Department. I observed him wearing a SAPD police uniform with police patches on each shoulder of his shirt. He was
wearing a police badge on the upper left portion of his chest and had a name tag displaying "Casillas" on his right
upper chest. Officer Casillas was wearing a black leather Same Browne belt, which included his duty 9mm Glock Gen

17 handgun (used on this incident). Officer Casillas is right-handed and had his duty weapon positioned on the right
side of his hip. Officer Casillas had his Body Worn Camera (BWC) mount positioned in the center of the uniform.
However, he had already docked the camera back into its assigned housing dock.
Senior Criminalist Williams examined and photographed Officer Casillas, his uniform, Sam Browne (firearm,
handcuffs, taser, magazine pouch, holster) and the contents of his pocket. The handgun contained (1) cartridge in the
chamber and (16) cartridges in the magazine. The magazine had a capacity of (17) cartridges.
The double magazine pouch located on Officer Casillas's Sam Browne only contained (1) magazine which housed
(17) 9mm caliber cartridges. The headstamp on all of the cartridges were "Hornady 9mm LUGER".

Sunday, December 1, 2024 - 2045 hours:
Santa Ana Police Department Homicide Section Detective Sergeant M. Lemmon #3174 notified me of an officer
involved shooting that occurred in the area of 200 N. Broadway in the city of Santa Ana. Sergeant Lemmon
requested that I respond to the scene in order to assist with the investigation.
1925 hours:
I arrived at the southern perimeter of the crime scene. Uniformed police officers in marked patrol units were providing
security for the crime scene, preventing access into the scene by unauthorized vehicular and pedestrian personnel.
2017 hours:
I checked into the scene with Santa Ana Police Department Patrol Officer A. Carrillo #3424, the designated scene
scribe.
2018 hours:
Santa Ana Police Department Patrol Sergeant L. Barragan #2937 conducted a briefing with detectives from the
Homicide Section and investigators from the California Department of Justice, regarding the initial response to the
scene and the preliminary investigation done by patrol personnel.
Sergeant Barragan informed us that on December 1, 2024, at approximately 1708 hours, Santa Ana Police
Department Communications received a call for service regarding a male armed with a shotgun at 1st Street and
Santa Ana PD 2024-26142 Report by Enriquez, David 3082 80 of 90
Spurgeon Street. Santa Ana Police Department Patrol Officers were initially unable to locate the suspect during an
area check. At approximately 1732 hours, communications received an additional call for service regarding a male
loading a rifle. At approximately 1733 hours, an officer involved shooting occurred on Broadway north of 1st Street.
Orange County Fire Authority (OCFA) Engine #75 personnel subsequently responded to the scene. The suspect,
identified as was transported to Orange County Global Medical Center where he was
ultimately pronounced deceased by Dr. at 1826 hours.
Patrol officers checked the area for surveillance footage and witnesses. Two witnesses were located, identified as
and and they were transported to the Santa Ana
Police Department.
Numerous items of evidence were located at the scene including a replica shotgun, expended cartridge cases, a
handgun magazine, and expended 40 mm less lethal ammunition.
Furthermore, Sergeant Barragan stated was involved in an incident involving a replica firearm with the
Santa Ana Police Department in the area of 3rd Street and Ross Street on November 26, 2024. The incident was
documented under Santa Ana Police Department case number 24-25754.
At the conclusion of the briefing, I was assigned to assist California Department of Justice Special Agent T. Baca
canvass the area for surveillance camera footage and witnesses.
2138 hours:
Special Agent Baca and I checked the following addresses for surveillance camera footage and witnesses:
102 N. Sycamore Street: Gus’ World Famous Fried Chicken- We contacted an employee, identified as She
attempted to allow us to review their surveillance camera system however, we were unable to access their system
due to technical difficulties. She advised us to come back later in the week when employees that were more familiar
with their surveillance system were present.
125 N. Broadway #218- We contacted a resident, identified as The following is a
summary of her statement. She advised she did not have any surveillance cameras at her apartment. In regards to
the shooting, stated she was in her apartment when she suddenly heard approximately four gunshots. When
she looked out of her window (that faces Broadway), she recalled seeing a police officer shooting a firearm several
times in the direction of the furniture store across the street from her apartment. She also recalled seeing the feet of
someone on the ground after the shooting. She did not see or hear anything prior to hearing the gunshots.
125 N. Broadway #217- We contacted a resident, identified as ). The following is a
summary of his statement. He advised he did not have any surveillance cameras at his apartment. In regards to the
shooting, stated he was in his apartment when he suddenly heard six to eight gunshots. When he looked out of
his window (that faces Broadway), he recalled seeing several police vehicles arrive on scene with their lights and
sirens activated. did not see or hear anything prior to hearing the gunshots.

Monday, December 1, 2024 - 1426 hours:
Detective F. Gomez #2706, Detective R. Sandoval #3466, and I met with investigators from the California Department
of Justice in the area of 1st Street and Broadway.
I was assigned to assist California Department of Justice Special Agent J. Genens canvass the area for surveillance
camera footage and witnesses.
1447 hours:
Special Agent Genens and I checked the following addresses for surveillance camera footage and witnesses:

211 N. Sycamore Street: Mobile Notary Public- We contacted the owner of the business, identified as
She allowed us to review her surveillance camera system. No surveillance footage of evidentiary value was
located.
201 W. Third Street: Public Parking Structure- We contacted an employee at the parking structure. He did not have
access to the surveillance cameras and he directed us to contact .
227 N. Broadway: Chapter One- We contacted the owner of the restaurant, identified as . He allowed
us to review his surveillance camera system however, no surveillance footage with evidentiary value was located.
125 N. Broadway: The Gypsy Den- We contacted an employee at the restaurant and she advised us she did not
have access to their surveillance system. She directed us to contact her manager .
220 W 1st Street: Centro Mobile- We contacted an employee at the business and he advised the business did not
