HomeMy WebLinkAboutOIS2012-0006 Hospital response - Ross_Redacted SA 12-026 Investigator: ANDREW ROSS Approved By: Date of Report: 10/23/12 Date: 10/24/12 Page 1 of 3 CASE NAME: GUILLEN, JAIME LOPEZ CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) TYPE: INITIAL RESPONSE/HOSPITAL AGENCY: SANTA ANA POLICE DEPARTMENT On October 22, 2012, at approximately 2230 hours, Orange County District Attorney (OCDA) Supervising Investigator RON SEMAN contacted me regarding the Santa Ana Police Department (SAPD) officer involved shooting of JAIME LOPEZ GUILLEN. SEMAN requested I respond to Western Medical Center – Santa Ana (WMCSA), located at 1001 North Tustin Avenue, Santa Ana, to handle the hospital investigation responsibilities. At approximately 2330 hours, I arrived at WMCSA emergency room. GUILLEN was confined to a hospital bed, handcuffed to the bed rail, inside Trauma Bay 1 (T-1), which was located in the emergency room department. GUILLEN was conscious and was engaged in conversation with the trauma nurse, RN . SAPD Officer DON KOSAKOWSKI was also present inside the trauma room with GUILLEN. On October 23, 2012, at approximately 0010 hours, SAPD Detective DEAN FULCHER arrived at WMCSA to assist with the investigation. At approximately 0030 hours, GUILLEN was transferred from the emergency room department to room 718, which was a single-bed custody unit located on the seventh floor of the hospital. ORANGE COUNTY DISTRICT ATTORNEY’S OFFICE BUREAU OF INVESTIGATION SUPPLEMENTAL REPORT SA 12-026 Investigator: ANDREW ROSS Approved By: Date of Report: 10/23/12 Date: 10/24/12 Page 2 of 3 At approximately 0105 hours, GUILLEN voluntarily signed an “Authorization for Release of Health Information” form, allowing the OCDA’s office to obtain a copy of GUILLEN’s medical records and pre transfusion/pre admitting blood samples. At approximately 0120 hours Orange County Crime Lab Forensic Specialist LINDSEY OLSON arrived at room 718. OLSON took 10 photographs of GUILLEN’s injuries. GUILLEN refused to provide a buccal swab. At approximately 0220 hours, OLSON concluded her investigation and left the scene. At approximately 0325 hours, OLSON returned to the WMCSA and took custody of GUILLEN’s property, which consisted of one pair of boxer shorts. The boxer shorts had been cut off GUILLEN by medical personnel and were blood stained. After OLSON took possession of the property, she left the scene. At approximately 0330 hours, SAPD Detective FULCHER and I concluded our investigation and left the scene. At approximately 1045 hours, I responded back to WMCSA and provided a copy of GUILLEN’s signed “Authorization for Release of Health Information” form to the WMCSA records department, requesting all documentation related to GUILLEN’s treatment. At approximately 1100 hours, I provided a copy of GUILLEN’s signed “Authorization for Release of Health Information” form to the WMCSA blood laboratory and took possession of GUILLEN’s pre transfusion/pre admitting blood. At approximately 1125 hours, I booked GUILLEN’s pre admitting/pre transfusion blood samples into evidence at the OCSD property and evidence unit.
