On the above date and time, I conducted an audio digital recorded interview of
Registered Nurse (RN), , in reference to a Santa Ana Police Department (SAPD)
officer involved shooting incident involving JESSICA GONZALEZ. has been a Registered
Nurse for eight years and is employed by the University of California, Irvine - Medical Center
(UCIMC). was on duty in the Emergency Room on June 23, 2013, assigned to a twelve
hour shift, starting at 0700 hours and ending at 1900 hours.
I provided with a copy of UCIMC’s Emergency Department’s Flow Sheet dated
June 23, 2012. All trauma patients that are unconscious when they are admitted to UCIMC’S
Emergency room, are given the name DOE, ISOLATE. confirmed that she had
completed the UCIMC’S Emergency Department’s Flow Sheet for a patient named, DOE,
ISOLATE, patient number #9066488 the morning of June 23, 2013. Patient DOE, ISOLATE has
now been positively identified as JESSICA GONZALEZ. The following is a summary of
statements:

On June 23, 2013, at approximately 0832 hours, was notified that GONZALEZ
was being transported to UCIMC with a gunshot wound, in full arrest and Cardiopulmonary
Resuscitation (CPR) was in process.
At 0850 hours, GONZALEZ arrived at the Emergency Room, Trauma Bay A.
described GONZALEZ as “very pale” and unconscious. The paramedics were performing CPR,
using a bag valve face mask for ventilations, and had already placed an interosseus needle in
GONZALEZ’S leg because they were unable to get peripheral intravenous access.
Emergency Medical Staff were unable to obtain a blood sample from GONZALEZ and did
not administer any medications. GONZALEZ was intubated and then Doctor
conducted a thoracotomy, opening up GONZALEZ’ chest, to determine if anything could be
done to repair the heart, and a chest tube was also inserted into GONZALEZ’ side. GONZALEZ’
heart continued to show no signs of cardiac activity during treatment.
At 0855 hours, GONZALEZ was pronounced deceased by UCIMC Medical
Staff moved GONZALEZ’ body from the Emergency Room to Douglas Hospital’s, bed 16 in order
to free the Trauma Bay for future emergency patients.
