On Tuesday, July 30, 2013, at approximately 1530 hours, Orange County District
Attorney Supervising Investigator (OCDA) RANDY LITWIN contacted me regarding an officer
involved shooting, involving the Santa Ana Police Department (SAPD). LITWIN requested I
respond to 622 South Harbor Blvd in the city of Santa Ana, and assist with the investigation.
At approximately 1610 hours, I arrived at the location and met with investigators from
the OCDA, and Detectives from SAPD, who were present to assist in the investigation.
At approximately 0718 hours, SAPD Acting Sergeant OLIVER GRANT #1500 conducted a
briefing of the known circumstances surrounding the incident, for the personnel involved in the
investigation.
Following the briefing, OCDA Supervising Investigator MARK GUTIERREZ assigned SAPD
Detective ANDREW ALVAREZ # 2857 and I to conduct an audio digitally recorded canvass of the
area for potential witnesses. ALVAREZ assisted in Spanish translation. I contacted the following
individuals:
ALEX’S ARCADE 626 South Harbor Blvd, Santa Ana, Ca 91707
On Tuesday, July 30, 2013, at approximately 1500 hours, was playing video games
inside Alex’s Arcade, located at 626 South Harbor Blvd in the city of Santa Ana. looked

outside the north door, of the Arcade because he thought one of the doors to his vehicle,
( ) was open. went outside to make sure his vehicle door was locked, and heard
the sound of a police vehicle horn. looked in a westerly direction, and saw a Santa Ana
police vehicle driving south bound from the west side of 604 South Harbor Blvd in the city of
Santa Ana. The SAPD vehicle continued southbound past the west side of the Arcade, and out
of sight. could not determine the gender of the police officer, who was driving the police
vehicle. returned back into the Arcade, and sat down at a video game.
Approximately twenty seconds later, heard a single gunshot. looked outside
the north door of the Arcade, and saw three pedestrians standing on the north sidewalk.
looked east and saw a male subject lying on the ground in front of 622 South Harbor Blvd in the
city of Santa Ana. saw two SAPD officers, administering medical aid to the male on the
ground. did not witness the shooting incident.

On Tuesday, July 30, 2013, at approximately 1500 hours, had just parked his
vehicle ( ) on the north side of Alex’s Arcade, located at 626 South Harbor Blvd in the
city of Santa Ana. As was exiting his vehicle, he saw several SAPD police officers
standing in front of 622 South Harbor Blvd in the city of Santa Ana. was told by the
SAPD officers to go back inside the arcade. did not witness the shooting incident.

On Tuesday, July, 30, 2013, at approximately 1300 hours, was inside Alex’s
Arcade, located at 626 South Harbor Blvd in the city of Santa Ana. ’ attention was drawn
to people gathering outside the north door of the Arcade. looked outside the north
door and saw a female SAPD officer standing in front of 622 South Harbor Blvd in the city of
Santa Ana. said the female officer looked scared. did not witness or hear the
shooting incident.

On Tuesday, July, 30, 2013, at approximately 1500 hours, was working at Harbor
Pharmacy, located at 646 South Harbor Blvd in the city of Santa Ana. looked outside and
saw SAPD officers, taping off the parking lot with crime scene tape. did not witness or hear
the shooting incident.
said there was a possibility that the store surveillance cameras might have captured
activity, immediately in front of the store. OCDA investigator BRAD TANNER was assigned to
retrieve any surveillance tapes from the store.

On Tuesday, July, 30 2013, at approximately 1400 hours, was working at the
Alpha Omega Christian Book Store, located at 640 South Harbor Blvd in the city of Santa Ana.
was working in her store when she heard one gunshot coming from outside.
went outside, and saw SAPD officers; standing in front of 622 South Harbor Blvd. did
not see or hear the shooting incident. took three photographs of the SAPD officers, on
her cellular telephone. I asked to provide me with the photographs.

SILVER COAST AUTO INSURANCE 638 South Harbor Blvd , Santa Ana, Ca 91707
On Tuesday, July, 30 2013, between 1500 and 1520 hours, was working at Silver
Coast Auto Insurance located at 638 South Harbor Blvd in the city of Santa Ana. While sitting at
his desk, saw a male Hispanic run past his store window, in a south bound direction.
described the subject as a male Hispanic, twenty two to twenty seven years old, 230 to
240 pounds, and 5ft 7 to 5ft 8 inches tall. The subject was wearing a baseball cap, dark T-shirt
and jeans. saw two SAPD police vehicles driving through the parking lot, at which time
he locked the front doors, and returned to his desk.
Approximately one to two minutes later, looked outside toward 622 South
Harbor Blvd, and saw a SAPD female officer run into the store. Seconds later, heard a
female’s voice say in English, “Get on the ground,” and then heard a gunshot. ’s
view was partially blocked by three pedestrians, who were standing on the north sidewalk in
front of Alex’s Arcade, located at 626 South Harbor Blvd in the city of Santa Ana. did
not witness the shooting incident.
