C-1-3252 3 BETWEEN 4 ORANGE COUNTY TRANSPORTATION AUTHORITY ' AND 6 CITY OF SANTA ANA 7 FOR a SENIOR MOBILITY PROGRAM 9 to THIS AMENDMENT NO. 1 is made and entered into this 28 Septemberday of . 2026("Effective 11 Date"), by and between the Change County Transportation Authority (hereinafter referred to as T2 "AUTHORITY'), and City of Santa Ana (hereinafter referred to as"CITY"). 13 WITNESSETH; 14 WHEREAS, by Cooperative Agreement No. C-1-3252 dated November 12, 2021, AUTHORITY 1= and CITY entered into a contract for the Senior Mobility Program(SMP)concerning senior transportation 16 services; and 17 WHEREAS, AUTHORITY requires a change to AUTHORITY's point of contact name; and is WHEREAS, AUTHORITY, at its sole discretion elects to exercise the Option Term of this 19 Cooperative Agreement up to an additional five (5) years, commencing July 1, 2026, through 20 June 30, 2031; and 21 WHEREAS, AUTHORITY's Board of Directors approved this Amendment No. 1 on 22 June 22, 2026; 23 NOW, THEREFORE, it is mutually understood and agreed by AUTHORITY and CITY that 24 Agreement No. C-1-3252 is hereby amended in the following particulars only: 25 1. Amend ARTICLE 3. RESPONSIBILITIES OF CITY Page 5 of 8, line 14, to delete 26 "Associate Contract Administrator's Name, Luis Martinez," and in lieu thereof insert "Contract Page 1 of 3 Rev pale.2Y{&w2i AMENDMENT NO. 1 TO COOPERATIVE AGREEMENT NO. C-1-3252 1 Administrator's name, Dara Doeum." 2 2. Amend ARTICLE 4. TERM OF AGREEMENT, Page 5 of 9, line 22, to delete 3 "June 30, 2026 (Initial Term)" as the expiration date of the Agreement and in lieu thereof insert 4 "June 30, 2031 (Option Term)." 5 3. Amend ARTICLE 5.
