Authorize the City Manager and Clerk of the Council to execute the attached amendment to the
agreement with Civic Collections, Inc. for an additional amount of $15,975 for total contract amount
of $115,975.
DISCUSSION
Civic Collections, Inc. (CCI) was the city's collection agency for delinquent dog license fees, penalties,
and new dog license fees. Under the terms of the agreement for the fiscal year ending June 30, 2012,
CC[ was to be compensated up to $100,000 for revenue collected. CCI collected additional revenue on
behalf of the City and therefore a final payment of $15,975 is still owed to the company.
An exhibit showing payments and payments due is attached "Exhibit A"
FISCAL IMPACT
Funds for the CCI contract are to be paid from revenue account (account no. 01110002 51003).

SIXTH AMENDMENT TO AGREEMENT
THIS SIXTH AMENDMENT TO AGREEMENT is entered into on this _ day
of , 2012 by and between Civic Collections, Inc., a California
corporation ("Contractor") and the City of Santa Ana, a charter city and municipal
corporation of the State of California ("City").
Recitals:
A. The parties entered into Agreement #N-2008-141, dated October 15, 2008,
(hereinafter "said Agreement") by which Contractor has provided dog license fee
collection services.
B. The parties entered into First Amendment to Agreement #A-2008-292, dated
November 17, 2008, in order to adjust the compensation of said Agreement.
C. The parties entered into Second Amendment to Agreement #A-2009-063, dated June
1, 2009, in order to adjust the compensation and extend the term of said Agreement.
D. The parties entered into Third Amendment to Agreement #A-2010-117, dated July 6,
2010, in order to extend the term of said Agreement.
E. The parties entered into Fourth Amendment to Agreement #A-2011-183, dated May
19, 2011, in order to adjust the compensation and extend the term of said Agreement.
F. The parties entered into Fifth Amendment to Agreement #A- 2011-183-01, dated
July 18, 2012, to amend the compensation of said Agreement to pay for additional
services that were provided during the term of said Agreement.
G. Thereafter, upon the expenditure of the allocated funds, the Parties agreed to increase
the compensation to pay for additional services provided by Contractor during the
term of said Agreement. However, due to an oversight and invoicing error, this
agreement was not formally executed in writing, although Contractor provided
additional services during the term set forth in said Agreement.
H. In accordance with the terms and conditions of said Agreement, the parties wish to
amend the compensation of said Agreement to pay for additional services that were
provided by Contractor during the term of said Agreement. Per agreement of the
parties, this Sixth Amendment to Agreement is now made to include all services
previously provided by Contractor to City in relation to the collection of dog license
collection services.

NOW, THEREFORE, in consideration of the mutual and respective promises, and
subject to all the terms and conditions of said Agreement, except as herein modified, the
parties agree as follows:
Section 2, COMPENSATION, shall be amended to increase compensation by an
amount not to exceed $15,975.91 to pay for additional dog license fee collection
services that were provided during the term of said Agreement. This agreement
shall include all services previously rendered by Contractor to City.
2. Except as hereinabove amended, all terms and conditions of said Agreement shall
remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have executed this Sixth Amendment to
Agreement on the date and year first written above.
