NOTICE IS HEREBY CYIVFN by Janet Robson to the CITY OF SANTA ANA (herefirafter
referred to as the "City") as fiallows:
1, A Consultant Agreeruent was entered into on January 1, 2015 between OC Safety, Inc. and
City (the, "Agreementl Whereby 0C Safety, Inc, would provide special skill and krwvvledgo
in the field of emergency first aid and CPRIARD training;
2. On or about June 30, 2015, all outstanding stook of OC Wety, Inc. was purohased by Janet
Robson from John La Placa as shower in the Resolution attached hereto as Exhibit "A"
3. As of June 30, 2015, Janet Robson become the director, shareholder, Secretary, Chief
Financial Officer and Chief Executive Officer of OC Safety, hm;
4. Janet Robson hereby agrees to honor all terms of the Agreement and it is attached hereto as
Exhibit "B" mid by this reference made a part hereof;
5� Janet Rribecui hereby accepts and assumes the righbk, liabilities and obligations of OC Safety,
Inc. under the Agreement from Inca 30, 2015 and maeafter;
6. Janet Robson ap

Agreement; arel
7. Janet Robson agrees to socept any and, all payment,; by the City pursuant to the Agreement
in the name of OC Safety, lnc.
