<br />7. IF THE CITY’S FORM IS NOT USED, REPORTS SHALL BE ON STRUCTURAL OBSERVER’S LETTERHEAD,
<br />STATE SITE ADDRESS, PLAN CHECK & PERMIT NUMBERS, STAGES & ELEMENTS OBSERVED, DATE
<br />OBSERVED, & COMPLETE CONTACT INFORMATION FOR STRUCTURAL OBSERVER.
<br />8. ALL STRUCTURAL OBSERVATION REPORTS, REGARDLESS OF FORM USED, SHALL INCLUDE THE
<br />LICENSE STAMP & SIGNATURE OF THE STRUCTURAL OBSERVER RESPONSIBLE FOR THE PROJECT.
<br />9. EACH STRUCTURAL OBSERVATION REPORT SHALL BE GIVEN TO THE OWNER OR OWNER’S
<br />REPRESENTATIVE, PROJECT CONTRACTOR, AND THE BUILDING INSPECTOR.
<br />10. THE CONTRACTOR SHALL RESOLVE ALL DEFICIENCIES & THE FINAL STRUCTURAL OBSERVATION
<br />REPORT ISSUED PRIOR TO FINAL INSPECTION OR ACCEPTANCE OF STRUCTURAL WORK BY THE
<br />BUILDING INSPECTOR.
<br />11. THE FINAL STRUCTURAL OBSERVATION REPORT SHALL STATE THAT THE STRUCTURAL SYSTEM
<br />CONFORMS TO THE APPROVED CONSTRUCTION DOCUMENTS & THAT ALL PREVIOUSLY OBSERVED
<br />DEFICIENCIES HAVE BEEN CORRECTED.
<br />12. FINAL INSPECTION OR OTHER ACCEPTANCE OF THE STRUCTURAL SYSTEM BY THE CHIEF BUILDING
<br />OFFICIAL, OR DESIGNEE, WILL NOT OCCUR UNTIL THE FINAL STRUCTURAL OBSERVATION REPORT IS
<br />RECEIVED.
<br />13. THE LICENSED DESIGN PROFESSIONAL IN RESPONSIBLE CHARGE SHALL PREPARE ALL CONSTRUCTION
<br />DOCUMENT CHANGES RELATING TO THE STRUCTURAL SYSTEMS. REVIEW & APPROVAL OF SUCH
<br />CHANGES BY THE CHIEF BUILDING OFFICIAL, OR DESIGNEE, SHALL BE OBTAIN BY THE DESIGN
<br />PROFESSIONAL AND/OR CONTRACTOR PRIOR TO INSTALLATION AND/OR CONSTRUCTION OF SAID
<br />CHANGES.
<br />1. AN APPROVED AGENCY, RETAINED BY OWNER AND SATISFACTORY TO STRUCTURAL ENGINEER AND
<br />GOVERNING CODE AUTHORITIES, SHALL PERFORM SPECIAL INSPECTIONS AND TESTS REQUIRED BY
<br />THE CONTRACT AND APPLICABLE CODE. THESE SPECIAL INSPECTIONS AND TESTS ARE IN ADDITION
<br />TO THE INSPECTIONS REQUIRED BY CBC SECTION 110 “INSPECTIONS”.
<br />2. APPROVED AGENCY SHALL KEEP RECORDS OF ALL SPECIAL INSPECTIONS AND TESTS AND SHALL
<br />FURNISH REPORTS OF SPECIAL INSPECTIONS AND TESTS TO THE GOVERNING CODE AUTHORITY AND
<br />THE STRUCTURAL ENGINEER. REPORTS SHALL INDICATE WHETHER THE WORK INSPECTED OR TESTED
<br />WAS OR WAS NOT COMPLETED IN CONFORMANCE TO APPROVED CONSTRUCTION DOCUMENTS.
<br />DISCREPANCIES SHALL BE BROUGHT TO THE IMMEDIATE ATTENTION OF THE CONTRACTOR FOR
<br />CORRECTION. IF NOT CORRECTED, THE DISCREPANCIES SHALL BE BROUGHT TO THE ATTENTION OF
<br />THE GOVERNING CODE AUTHORITY AND THE STRUCTURAL ENGINEER PRIOR TO THE COMPLETION OF
<br />THAT PHASE OF WORK. A FINAL REPORT DOCUMENTING REQUIRED SPECIAL INSPECTIONS AND TESTS,
<br />AND CORRECTION OF DISCREPANCIES, SHALL BE SUBMITTED UPON COMPLETION OF THAT PHASE OF
<br />WORK.
<br />3. WHERE FABRICATION OF STRUCTURAL, LOAD-BEARING OR LATERAL LOAD-RESISTING MEMBERS OR
<br />ASSEMBLIES IS BEING CONDUCTED ON THE PREMISES OF A FABRICATOR’S SHOP, SPECIAL INSPECTION
<br />OF THE FABRICATED ITEMS SHALL BE PERFORMED DURING FABRICATION. SPECIAL INSPECTIONS
<br />DURING FABRICATION ARE NOT REQUIRED WHERE WORK IS DONE ON THE PREMISES OF A
<br />FABRICATOR REGISTERED AND APPROVED BY THE GOVERNING CODE AUTHORITY TO PERFORM SUCH
<br />WORK WITHOUT SPECIAL INSPECTION. FABRICATOR APPROVAL SHALL BE IN ACCORDANCE TO CBC
<br />SECTION 1704.2.5.1. ACCORDING TO 1704A.2.5.1 FABRICATOR APPROVAL IS NOT PERMITTED BY
<br />DSA-SS, DSA-SS/CC, OR OSHPD.
<br />4. EACH CONTRACTOR RESPONSIBLE FOR THE CONSTRUCTION OF A MAIN WIND OR SEISMIC FORCE
<br />RESISTING SYSTEM, DESIGNATED SEISMIC SYSTEM, OR A WIND OR SEISMIC FORCE-RESISTING
<br />COMPONENT LISTED IN THE STATEMENT OF SPECIAL INSPECTION SHALL SUBMIT A WRITTEN
<br />STATEMENT OF RESPONSIBILITY TO THE GOVERNING CODE AUTHORITY AND THE OWNER PRIOR TO
<br />THE COMMENCEMENT OF WORK ON THE SYSTEM OR COMPONENT. THE CONTRACTOR’S STATEMENT OF
<br />RESPONSIBILITY SHALL CONTAIN ACKNOWLEDGEMENT OF AWARENESS OF THE SPECIAL
<br />REQUIREMENTS OF INSPECTION.
<br />5. CONTRACTOR SHALL SUBMIT MATERIAL CERTIFICATION OR LABORATORY TEST REPORTS CERTIFYING
<br />MATERIALS ARE OF IDENTIFIABLE TESTED STOCK, COMPLYING WITH PROJECT SPECIFICATIONS, TO
<br />OWNER, APPROVED AGENCY, STRUCTURAL ENGINEER AND, UPON REQUEST, TO GOVERNING CODE
<br />AUTHORITY. IF LABORATORY TEST REPORTS CANNOT BE MADE AVAILABLE, APPROVED AGENCY WILL
<br />PERFORM TESTS AS DIRECTED BY STRUCTURAL ENGINEER. CONTRACTOR SHALL PAY FOR COSTS
<br />RELATED TO TESTS AND INSPECTIONS OF UNIDENTIFIABLE MATERIALS, MATERIALS FURNISHED
<br />WITHOUT LABORATORY TEST REPORTS, MATERIALS FOUND DEFICIENT AFTER INITIAL TESTS AND

<br />a. THE FIRST TWO COATS SHALL BE AS REQUIRED FOR THE FIRST COATS OF EXTERIOR PLASTER,
<br />EXCEPT THAT THE MOIST-CURING TIME PERIOD BETWEEN THE FIRST AND SECOND COATS SHALL
<br />BE NOT LESS THAN 24 HOURS. MOIST CURING SHALL NOT BE REQUIRED WHERE JOB AND WEATHER
