<br /> <br /> <br /> These photos shall also certify that the placement and geometry of the <br />equipment on the mount is as depicted in the antenna placement diagram in this <br />form. <br />o Photos that show the model number of each antenna and piece of equipment installed <br />per sector. <br /> <br />Antenna & equipment placement and Geometry Confirmation: <br /> <br />• The contractor shall certify that the antenna & equipment placement and geometry is in <br />accordance with the sketch and table as included in the mount analysis and noted below. <br /> <br />☐ The contractor certifies that the photos support and the equipment on the mount is as depicted on <br />the sketch and table included in this form and with the mount analysis provided. <br /> <br />OR <br /> <br />☐ The contractor notes that the equipment on the mount is not in accordance with the sketch and has <br />noted the differences below and provided photo documentation of any alterations. <br /> <br /> <br />Special Instructions / Validation as required from the MA or any other information the contractor <br />deems necessary to share that was identified: <br /> <br />Issue: <br />Contractor shall shift mount pipes horizontally in position 1 on all sectors such that it is 24” from position 2 pipe. <br />Reconnect shifted pipes to mount horizontals using existing connection crossover plates, etc. with new bolting <br />hardware. Do not reuse existing bolts. <br /> <br />Contractor shall install a new 108” long PIPE 2.5 SCH40 mount pipe position 1 (in all sectors). Install 24” from relocated <br />position 1 pipe. Top of pipe shall be 36” above top face horizontal (match relocated position 1 pipe location on mount). <br />Attach using VZWSMART MSK1 crossover plates.
