KICK-OFF MEETING <br />Priorto the start of the PMect, we Will have a kick-off <br />mooing With City Staff and the PrQIa0t team to review the <br />project objeotivosi schedule, the scope and approach. <br />At this meWr% We will 6stablishe point of contact for <br />the project and requestAff avallable information related <br />to the project. In addition. the WS Protect Manager Will <br />coordi nets all aspects of the Project And, manage the <br />schedule and costeto ensure the projeces completion to <br />the $11fisfacdon Of the city, <br />B� DATA COLLECTION AND PROJECT <br />DOCUMENTATION <br />We Will research all Information Pertinent to the project <br />such it$, the latest street improvement plans, right of way <br />maps, previous Initorsootions and median in'lProvditOnt <br />plans,storm drain, water andsewer plans, and Witty Plans <br />inorder to include this information in the project plans. <br />0. DESIGN SURVEY <br />The survey tasks include, but are not limited to, the <br />followhis <br />Wqwfil perform a topographic survey and prepare base <br />mapp. We will make sure our bast maps show the <br />tight of way, curb and gutter, sidewalk, WWWAY, street <br />centerline, access rOmPg� driveways, signs, poles, otc. <br />In addition, we will Include all Irflormation obtained from <br />all affected utifitycompanle$ within the roadway and <br />parkways, Ths-base Map$ will then be confirmed during <br />out field walk& The main objective 16 to have all existing <br />Impvmmonta scovrately depicted in our bass maps so <br />that all constraints are considered during our design of the <br />project. The survaywill Include cross sections the street to <br />adequately show the condition of me project site prior to <br />constructiom <br />D. UTILITY INVESTIGATION AND COORDINATION <br />We will research and establish record location of all <br />utilities and u011ty easements, as nftessarY- W1 Ity <br />coordination on the project will be ongoing and Will <br />generally consist of the following sub tasks: <br />Notify each utility purveyor, in writing, of the City's <br />proposed plans and schedule and request copies <br />of record drawings or atias maps of existing and <br />proposed facilities within the Pmject boundary. <br />• obtain existing dry and,wet utilityplans/records within. <br />the project rimits. it also Includes the coordination of <br />the adjustment of any utilities per oliy or other agency <br />standards. Once the plans are completed we will send <br />them to all utility companies affected by the Pwiedt. <br />• COonfinVIS all utilitles and ot>tain approval in writing <br />from affected utility companies regarding conflicts, <br />relocations and Improvements. <br />• Access the Underground Service Alert!s webs.1te and <br />carnpile a list of member agencies that Own lavflfts <br />withlitthe project 11mits, <br />• Augment and/or reline ths- above (let based on <br />available Information at the City Records DOPArtraent <br />and from field Investigottlon. <br />• Utilities shown, on the, r.eo.old drawings, will be surface <br />varffied, and added to the base MOP, along With the <br />most recent easement lines shown for each facility. <br />• Ideraffyiriterfaces between existing foollitles and <br />future prqjeot construction, <br />• Consuft With the service planners and others as <br />necessary within each utility company to resolve any <br />oonflicle. <br />• Keep City staff informed on all discussions with Utility <br />companies via written meeting minutes or records of <br />telephone conversation, <br />• Prepare a Utility Notification Log 0 track utility <br />company contacts and responses including corylact <br />Information and dates of all outgoing and incoming <br />correspondence, <br />E, PAVEMENT EVALUATION <br />We will provide goolecholcal Investigation, <br />recomm . ondations, and reports to determine the exbrt <br />rehabilitation methods Including but not to limited to <br />reconstruction areas and the proper pavermfft thickness <br />thr reconstruction areas and overlay. We will perform <br />a geateohnical reconnaissance of the site slid take Lip <br />to six borings samples at strategic locations within the <br />project Sees to observe the existing pavement thickness <br />and soil conditions beneath and to facilitate collection of <br />sub -grade soff samples for lab testing. The results of our <br />told and laboratory testing Will be provided In a report <br />which will Include; thickness of Pavement section, site soil <br />obaraQterization, R-Value of sub -grade soll% and pavement <br />improvements recommendations, <br />F.
