the grant period, Brighter Futures for Children of Prisoners will achieve the following goals and outcomes: <br />• Recruit a minimum of 45 new Santa Ana children of prisoners and provide comprehensive services to <br />them, to their guardians and to their siblings, for a total of 180 Santa Ana residents served. <br />• Sustain a minimum caseload of 175 mentor/mentee matches, plus their families for a total of 500+ people served; <br />• 85% of mentors will honor the one-on-one outings of 8 hours per months for 12 months and turn in their required <br />online monthly activity forms; <br />• Provide ongoing wraparound services to the mentee families; <br />• Conduct a series of 5 nutrition seminars for caregivers and mentees; <br />• 50 mentees will attend the "College Is For Me Too!" camp; <br />• 85% of mentees attending the camp will report they look forward to attending college; <br />• 65%of mentees will attend at least 2 STEM events and develop an interest in science -related careers. <br />• 85% of mentees will improve their behavior and attitudes, which will be noted on the monthly activity notes, teacher & <br />parent comments, and through surveys; and <br />• 90% of mentees will take advantage of new opportunities and avenues for positive use of time; positive self-expression; <br />and civic engagement. Mentees will also show increased self-esteem, morale, a sense of responsibility, increased <br />interpersonal skills, and decision -making skills. <br />• 100% of mentees will receive exciting, age -appropriate books on an ongoing basis. <br />�ograms are evaluated using science -based tools utilizing standardized and locally developed measures. Data collection <br />cludes, but is not limited to: Mentee and Mentor demographic data (age, gender, ethnicity); Household income of mentee <br />milies; Tracking grid of wrap -around services provided; Caregiver post surveys for Caregiver Education Seminars; Mentee post <br />arveys for "College Is for Me Too!" Summer Education Camp and STEM workshops to measure attitudes and knowledge gained; <br />ttendance records for Caregiver Education Seminars, Summer Education Camp and STEM workshops; Duration of <br />ientor/mentee matches; Caregiver and mentee satisfaction surveys — utilizing the U.S. Department of Health and Human <br />=rvices Family and Youth Services Bureau's Relationship Quality Survey Instrument. <br />• Quantifiable results - We track the number of youth and families served, mentor/mentee matches and amount of one- <br />on-one outings completed, and the total number of youth and families served in each activity and in the program overall. <br />• Reporting format - We utilize pre and post- surveys, which include multiple choice and narrative questions to allow <br />participants to express their satisfaction levels as well as voice any concerns or praises. <br />• Mentors are required to submit monthly online activity forms where they outline the contact and activities completed <br />with their mentee and detail any successes or concerns they are having with mentees, ask questions, or provide general <br />feedback <br />EXHIBIT A <br />
