One major benefit is that clients can learn <br />about and enroll in a variety of programs through a single visit via the One -e -App electronic application portal. In <br />2017 CHIOC completed over 6,000 applications for individuals in the county. <br />Retention and Utilization are addressed through Care Connections case management. Building upon <br />relationships with each individual or family, CECs help clients navigate and utilize available services and benefits <br />once enrolled. CHIOC assists with verifying enrollment, selecting a provider, scheduling an initial health <br />assessment, and retaining coverage. <br />CHIOC's Community Health Access Program is currently available Monday through Saturday with varied hours to <br />best accommodate clients. In addition to regular office hours, we engage in 10 to 20 outreach presentations and <br />events each month; these are generally held on evenings and weekends. <br />Services are typically provided in a one-on-one setting. However, CHIOC's CECs are available by phone for <br />questions and assistance, and we also offer health literacy workshops, which often take place in <br />group/classroom settings at the CHIOC office or local community centers. Services may also be provided during <br />outreach at community events and health fairs. <br />EXHIBIT A <br />
