• After -School Tutoring and College Readiness—We believe that youth education is a key stepping stone towards <br />breaking the cycle of poverty. Our after-school programming provides youth with holistic out -of- classroom education <br />enrichment and college readiness services that are normally available to their more affluent peers. Project Access <br />integrates experiential learning, community partnerships, healthy habits and behaviors, rigorous STEM learning <br />activities, college empowerment, and character -building opportunities into the program's curriculum. Our onsite <br />presence provides families with unparalleled access to high-quality after-school programming that sets youth on a path <br />toward a lifetime of opportunities. <br />The goal of the program is to empower adults to achieve economic stability, promote youth academic achievement, <br />and assist families with reclaiming their right to economic stability. Our wraparound services stack financial literacy, <br />community -building, and youth services with the Family Resource Model so that the residents we serve receive high- <br />quality, immediate care that sets the stage for long-term self-sufficiency. <br />Project Access anticipates achieving the following outputs for the program during the grant period: <br />• Engage 150 unduplicated low-income, at -risk youth with structured tutoring, out -of -classroom enrichment, <br />college campus visits, educational field trips, health and wellness knowledge -building, and character <br />development activities. <br />• 750 family members will attend a Project Access community -building event, including engaging workshops, <br />community dinners, cultural celebrations, or movie night. <br />• 75 low-income adults will participate in Project Access financial literacy workshops, financial capacity - <br />building activities, ESL classes, or technology training. <br />Project Access is committed to achieving the following objectives during the grant period: <br />• 70% of participating youth will demonstrate positive academic achievement in Reading/English and/or <br />Math, as evidenced by school report cards. <br />• 90% of teens will report that Project Access programming helped them achieve an increased sense of self-esteem. <br />• 80% of participating families will report that their children's access to fruits and vegetables increased as a result of <br />Project Access programming. <br />• 90% of the residents will indicate that the assistance they received was helpful in securing <br />employment. <br />• 90% of seniors will report that Project Access has helped them improve a current life situation. <br />Key milestones for the program during the City of Santa Community Development Block Grant Period include: <br />• STEM will be folded into daily youth summer programing. <br />• Daily execution of ongoing after-school tutoring and enrichment services. <br />• A community health fair that takes place during the summer months. <br />• Ongoing weekly teen character -building and leadership activities. <br />• Annual tours of local university campuses. <br />performance Measures <br />We employ one full-time and one part-time social impact analyst positions to measure and evaluate the success of our <br />programs. The program evaluation team will collect monthly data to measure progress against goal and track the number <br />of youth served on a consistent basis. Project Access monitors program success of the Economic Empowerment and <br />Family Support Program using pre- and post-tests, observational tools, a technology skills assessment, the annual Teen <br />Survey, and Community Surveys. Surveys indicate how our programs and services have changed residents' lives as a result <br />of learned skills, behaviors, or values. Surveys also measure an increase in knowledge or access to critical resources. <br />Additionally, Project Access collects report cards for youth to measure increased academic achievement. <br />EXHIBIT A <br />
