STATEMENT OF QUALIFICATIONS
<br />ERIK KLUNK I PUBLICWORKS INSPECTOR/TECHNICIAN
<br />Mr. Erik Klunk has a more than 14 year career providing special inspection services
<br />for public and private sector clients. Erik's assignments have included responsibilities
<br />for ensuring quality control, performing testing and inspection, accurately reading
<br />plans and specifications, and being knowledgeable of building codes and remaining
<br />up-to-date on changes.
<br />RELEVANT PROJECT INVOLVEMENT / EXPERIENCE
<br />MOJAVE VALLEY AREA OFFICE I CALIFORNIA HIGHWAY PATROL, MOJAVE
<br />Special Inspector I Mr. Klunk provided DSA masonry wall inspection as well as
<br />special inspection of field welding of structural steel for this project. This project
<br />was designed and built to meet the Essential Services Act ( ESA) requirements and be
<br />certified by Division of State Architect (DSA). The single -story building incorporates
<br />public areas, main office space, and private offices. RMA Group's services included
<br />compaction testing of subgrade during grading, observation of CMU placement,
<br />with testing of mortar cylinders used in masonry construction; concrete sampling
<br />and testing; and shop and field welding inspection for embeds and columns.
<br />MONTE VISTA AND 11TH STREET, UPLAND
<br />Public Works Special Inspector / Technician I Mr. Klunk served as Special Inspector for the Monte Vista and 11th Street Project
<br />located in the City of Upland. RMA Group's scope of work consisted of performing inspection of foundation excavations, compaction
<br />testing, and sampling and testing of reinforced concrete.
<br />INTERSTATE 10, ETIWANDA AVENUE INTERCHANGE, ONTARIO & FONTANA
<br />Public Works Special Inspector/Technician I Mr. Klunkserved as lead testing technician responsible fortesting and inspection of soil,
<br />aggregate base, asphalt concrete, and Portland Cement Concrete. Erik prepared daily test reports and performed materials testing
<br />in accordance with job special provisions, Caltrans Standard Specifications and Caltrans Construction Manual. He has performed
<br />laydown inspection that involved observing spreading operations, compaction, smoothness, and joint locations and offsets. He was
<br />also responsible for tracking asphalt theoretical quantities against actual tonnage.
<br />INLAND EMPIRE UTILITIES AGENCY ON-CALL, VARIOUS LOCATIONS
<br />Public Works Special Inspector / Technician I Mr. Klunk acts as Special Inspector for this contract. He provided inspection with
<br />detailed documentation that included description of the work in progress by each trade or consultant, approved submittals, change
<br />orders, bulletins, requests for information or clarification, and correction notices or notices of non-compliance issued. RMA Group
<br />has been providing construction and welding inspection, soil engineering during design and construction, and materials testing
<br />for the past 17 years to the Inland Empire Utilities Agency. During this time, RMA Group worked on all five of this agency's water
<br />treatment plants, the RP Administration and Operation Building, Peaker Plant, and Wetland Restoration Project.
<br />1-405 / SEPULVEDA PASS, LOS ANGELES
<br />Public Works Special Inspector/ Technician I Mr. Klunk served as Special Inspector for the 1-405 / Sepulveda Pass Project. The project
<br />consisted of adding a 30 -mile high -occupancy vehicle (HOV) lane on northbound 1-405 between the 1-30 and US 101 Freeways,
<br />removal and replacement of three bridges, realignment of 27 on- and off -ramps, widening of 13 existing underpasses, and structures
<br />to reduce traffic congestion.
<br />CRENSHAW / LAX LRT, CRENSHAW
<br />Public Works Special Inspector/ Technician I Mr. Klunk provides special inspection services for this transit project that constructs an
<br />8.5 -mile light-rail line that will run between the Expo Line on Exposition Boulevard and the Metro Green Line serving the Crenshaw
<br />Corridor, Inglewood, Westchester and the LAX area with eight stations, a maintenance facility, park -ride lots, and five power
<br />substations. RMA Group is responsible for the supervision and coordination of inspections and testing on a daily basis, primary review
<br />inspection and testing records, and implementation of the Quality Program Manual. To provide test results in a timely manner, RMA
<br />has provided an on-site laboratory approved by LA City, and AASHTO certified.
