Early Bids: Bid proposals received by the City of Santa Ana, Purchasing Division <br />prior to the Submission Due Date will be date and time stamped and secured by <br />the City of Santa Ana, Purchasing Division; <br />D. Title Page: All bid proposals must be clearly marked on the outside binder cover <br />with a title page containing the following information: <br />i. Name of ambulance transportation service operator (i.e., the Proposer); <br />ii. Name of RFP and RFP #18-059; <br />iii. Proposer address and telephone number; <br />iv. Proposer authorized contact person; <br />v. Proposer authorized contact person's signature; <br />vi. Bid proposal submission date; and <br />vii. Marked as original submitted copy <br />E. Bid Format <br />To facilitate the bid proposal review process, all bid proposals must strictly <br />adhere to the format, table of contents, titles, page limits, and numbering for <br />requested information items as set forth in Section 7, Bid Proposal Submission <br />Forms. Any deviation from the Section 7 format and the required Bid Proposal <br />Submission Forms may result in disqualification, at the City's discretion. All <br />Proposers must strictly adhere to the following rules in preparing their bid <br />proposals, and any deviation may result in disqualification: <br />i. Use only standard one inch (1 ") margins; <br />ii.Use only size 12 font; <br />iii. Use only single -sided pages; <br />iv. Observe all page limit requirements, as specified; <br />v.
