4. LIST OF SUBCONTRACTOR (ATTACHMENT W: Proposals shall include a listing, <br />subcontractors that will perform work on this project. <br />5. CERTIFICATIONS/STATEMENTS <br />The following forms, included in Attachment 3 in the Appendix of this RFP shall be signed <br />and included as part of the proposal submittal package: <br />f Attachment 3-1: Non -Collusion Affidavit <br />• Attachment 3-2: Non -Lobbying Certification <br />• Attachment 3-3: Non -Discrimination Certification <br />• Attachment 3-4: Agreement Statement <br />V. PROPOSAL REVIEW (CONSULTANT SELECTION <br />The CITY has established a proposal review committee to evaluate proposals based on the <br />response to the RFP, which includes adherence to outlined directions and format, and the CITY <br />evaluation criteria set forth above. A final score will be calculated for each submitted proposal <br />and used to rank the PROPOSERS. <br />The criteria for evaluating the proposals submitted will take the following items into consideration: <br />• Finn/Team Experience 25% <br />• References/Relevant Project Experience 15% <br />• Manner for Performing Service/Schedule 40% <br />• Fee 20% <br />The CITY reserves the right to request clarification and/or request additional information from <br />PROPOSERS, if necessary. Such clarifications and/or additional information shall be submitted <br />by the PROPOSERS as an Addendum to the Proposal upon request of the Contract Officer. <br />However, since PROPOSERS may not request to provide additional input, PROPOSERS are <br />advised to submit complete information in the Proposal. <br />Based on the evaluation results, finalists may be asked to participate in an interview session and/or <br />supplemental questionnaire the week of February 4, 2019. Exact date and time will be provided <br />to the finalist(s) seven days prior to the assigned interview session and/or due date of the <br />supplemental questionnaire. If an interview session and/or supplemental questionnaire is <br />performed, the results will be added to the initial evaluation score to determine the selected <br />company. <br />B. SELECTION <br />The CITY will recommend award of the agreement to the PROPOSER who will provide the best <br />quality and value to the CITY.
