SSA — Eddy Surya, Staff Specialist or designee (FUP-Eligible Families) <br />Silva Osorio — Reyes, Senior Social Worker or designee (FUP-Eligible Youth) <br />3. Continuum of Care (CoC) — Jim Wheeler, Continuum of Care Manager or designee <br />VIII. SSA RESPONSIBILITIES <br />As the PCWA, the Children and Family Services (CFS) Division of SSA provides <br />child welfare services for the entire County and is responsible for compliance with <br />the provisions of this MOU and the following (for both FUP-eligible families and <br />youth unless otherwise stated) <br />1. Participation <br />a. Meeting at least quarterly with SAHA and the CoC. <br />b. Upon award and thereafter as needed, training of SAHA staff on referral <br />policies/procedures. <br />c. Provision of sufficient staffing to insure: <br />i. FUP-eligible households are identified in a timely manner. <br />ii. Referrals are provided to SAHA within 30 days of receiving notification <br />from SAHA of voucher availability. <br />2. Outreach and Identification of FUP-Eligible Households <br />a. System SSA will continue to utilize the established FUP referral and <br />assessment process. <br />b. Monthly assessment of caseloads to identify households that are <br />potentially eligible. <br />c. For those FUP-eligible youth in the community who are no longer part <br />of SSA's active caseload, SSA will refer those youth to the <br />Orangewood Foundation for housing support services, including <br />emergency and on-going case management. <br />d. Engagement of households referred by SAHA, the CoC, other <br />community partners, and Coordinated Entry to determine potential <br />eligibility to SSA services and/or the FUP. <br />3. Referral <br />Referrals are made for priority families as soon as they are identified (not contingent <br />upon successful completion of a case management plan). <br />a. Prioritization of referrals <br />i. Priority 1 - Family maintenance <br />ii. Priority 2 - Imminent (within three (3) months) reunification <br />iii. Priority 3 - Reunification (within six (6) months) <br />iv. Priority 4 — Youth <br />b. At the time, the referral is submitted to SAHA, provide written verification: <br />i. The household is FUP-eligible based upon the criteria established <br />in Section 8 (x) of the United States Housing Act of 1937, and the <br />Family Unification Notice of Funding Availability for Fiscal Years <br />2017 and 2018. <br />ii. The jurisdictional requirement is met (see Section IV, Population to be <br />Served). <br />4. Supportive Services <br />a. Housing Search Assistance (including in low -poverty census tracts) - The <br />PCWA's social work staff who work directly with families involved in the child <br />welfare system will assist families to locate and secure housing. Social workers <br />will aid families with navigating resources offered through Orange County's 2-1-1 <br />system and local Housing Authority organizations. These resources include <br />housing search assistance for FUP eligible families and individuals. SSA will use <br />the Families and Communities Together (FaCT) coalition platform of 15 Family <br />Page 3 of 7 <br />3-6 <br />
