EXHIBIT 1 <br />IV. CAPACITY <br />In 2018, SAHA and SSA will establish a partnership to implement FUP. Over the course of <br />the partnership, eligible households will successfully be housed as a result of the services <br />and assistance provided via the partnership. <br />V. DEFINITIONS <br />1. FUP - Eligible f=amily is defined as a family that SSA has certified as a family for whom <br />the lack of adequate housing is a primary factor in the imminent placement of the <br />family's child, or children, in out -of -home care, or in the delay of discharge of a child, or <br />children, to the family from out -of -home care, and that SAHA has determined is eligible <br />for an HCV. <br />2. FUP — Eligible Youth is defined as a youth that SSA has certified to be at least 18 years <br />old, and not more than 24 years of age, and who has left foster care, or will leave foster <br />care within 90 days, in accordance with a transition plan described in section 475(5)(H) <br />of the Social Security Act, and is homeless or is at risk of becoming homeless at age 16 <br />or older. <br />3. Lack of Adequate Housing <br />a. Living in substandard or dilapidated housing; <br />b, Homeless; <br />c. Displaced by domestic violence; <br />d. Living in an overcrowded unit; or <br />e. Living in housing not accessible to an applicable children) due to the <br />nature of the disability. <br />4. Substandard or Dilapidated Housing <br />a. Does not provide safe and adequate shelter and endangers the health, <br />safety or well-being of a household; <br />b. Does not have operable or usable plumbing; <br />c. Has unsafe, inadequate, or no electricity and/or heat; <br />d. Should, but does not have a kitchen; or <br />e. Has been declared unfit for habitation by an agency or unit of government. <br />5. Homeless <br />a. Lacking a fixed, regular, and adequate nighttime residence. This <br />includes living in a temporary shelter or motel/hotel, <br />b. Has a primary nighttime residence that is: <br />I. A shelter designed to provide temporary living accommodations; <br />fl. An institution that provides a temporary residence for persons <br />intended to be institutionalized; or <br />iii. A place not designated for, or ordinarily used as a regular sleeping <br />accommodation for human beings. <br />6. Overcrowded <br />a. A household is separated from its minor child(ren) and the parent(s) is/are <br />living in an otherwise standard housing unit, but, after the household is <br />reunited, the parent's housing unit would be overcrowded for the entire <br />family and would be considered substandard; or <br />b. The household is living with its child(ren) in a unit that is overcrowded for <br />the entire household and this overcrowded situation may result in the <br />imminent placement of the child(ren) in out -of -home care. <br />c. A "youth" household is living In a unit that is overcrowded. <br />VI. POPULATION TO BE SERVED <br />Households that meet the eligibility criteria in Section f, Background that live and/or work <br />within SAHA's jurisdiction. <br />Page 2 of 7 <br />3-5 <br />
