EXHIBIT 1 <br />Resource Centers (FRCs) throughout Orange County and funded by the Orange <br />County Social Services Agency, FaCT FRC's offer a multitude of services <br />including emergency assistance funds for rental assistance, utility payments, <br />security deposits, and moving costs. FRCs are also funded with California's <br />Work Opportunity and Responsibility to Kids (CaIWORKs) dollars in through the <br />Family Stabilization program to assist families in crisis, which includes financial <br />assistance for homeless families. <br />b. Case management (which includes service plans, and referrals to resources to <br />meet needs not directly met by SSA) <br />i. FUP families that have been issued a voucher will have an assigned SSA <br />social worker. The families' social workers will provide ongoing case <br />management which includes developing a case plan based on the family <br />needs and will involve identifying both housing -related and non -housing <br />related needs. SSA social workers will maintain regular monthly contact with <br />the family and ongoing case management, SSA social workers will make <br />referrals to services in the community or to contracted service providers for <br />the family to follow up with and get connected to. <br />ii. If it is determined that an FUP family can benefit from additional case <br />management services beyond the timeframe that the SSA social worker has <br />the case, a referral will be made to the Family Resource Center where the <br />family will be assigned to a Family Support Specialist who will provide them <br />with ongoing case management and support. <br />iii. For youth who will leave foster care within 90 days and are homeless, or <br />foster youth who are at risk of becoming homeless at age 16 older, the <br />Orange County PCWA will designate a PCWA social worker to perform the <br />following tasks: identify, on a continuous basis, FUP-eligible youth through <br />outreach to and referrals from all local governmental (e.g., child welfare <br />services, behavioral health services, secondary and post -secondary <br />education, etc.) and nongovernmental organizations (e.g., Court Appointed <br />Special Advocates, transitional housing providers, youth outreach providers, <br />Family Resource Centers, etc.) serving current and former foster youth. Refer <br />those youth to the Orangewood Foundation for housing assistance services <br />and emergency and on-going case management. <br />iv. Timely follow-up supportive services after a youth leases a unit. <br />c. Cash assistance - The CFT Discretionary Fund will be utilized by SSA to <br />assist FUP-eligible families lease -up with a FUP voucher. In addition, SSA <br />will continue to utilize a combination of existing programs and partnerships <br />with community based organizations, faith based organizations, and <br />contracted providers, including the 15 Families and Communities Together <br />Family Resource Centers. For youth in need of housing, cash assistance can <br />be obtained through the Orangewood Foundation through a blend of privately <br />donated funding and a Cal OES grant. <br />d. Liaison between household and SAHA <br />e. Crisis Intervention <br />IX. SARA RESPONSIBILITIES <br />As the PHA, SAHA provides rental assistance payments and is responsible for <br />compliance with the provisions of this MOU and the following (for both FUP-eligible <br />families and youth unless otherwise stated): <br />1. Program Administration and Operation <br />a. Meeting at least quarterly with SSA and the CoC. <br />b. Funding application and annual reporting processes. <br />c.
