EXHIBIT 1 <br />1. Meeting at least quarterly with SAHA and SSA. <br />2. Cooperating and assisting SAHA and SSA with administration of the program. <br />3. Integration of the prioritization and referral process for FUP-elig€ble youth into the CoC's <br />Coordinated Entry process via screening and referral by outreach teams, <br />4. Identification of services to be provided using CoC program funds to families and youth <br />who qualify for CoC program assistance utilizing existing Coordinated Entry <br />policies/processes/procedures and Orange County's 2-1-1 system. <br />XI. COLLABORATION <br />In compliance with HUD regulations, SAHA,SSA and the CoC will collaborate by <br />communicating regularly, and meeting at least quarterlyto most effectively administer <br />FUP. Further, each will cooperate with each other and HUD in the following: <br />1. Compliance with the provisions of this MOU <br />2. Data collection and reporting <br />3. Applications for funding <br />4. Annual reporting processes <br />5. Reviews/Audits <br />6. Program evaluation <br />7. Additional FUP Projects <br />XII. GOALS AND STANDARDS OF SUCCESS <br />1. Accurate and timely issuance of HCVs to the target population, as defined in Section VI <br />"Population to be served". <br />2. Utilization of at least 90% of HCVs at all times. <br />3. Increase in FUP assisted household participation in the Family Self Sufficiency Program. <br />4. Most effective use of CoC Coordinated Entry System by directing eligible households to <br />most appropriate resource. <br />XIII. CONFIDENTIALITY: <br />1. SAHA, SSA and the CoC agree to maintain the confidentiality of all records and <br />client information, pursuant to all applicable case law, federal and/or state laws <br />and regulations inducting without limitation, U.S. Privacy Act of 1974 and the State <br />of California Welfare and Institutions Code sections 827 and 10850 et seq,, and <br />Orange County Juvenile Court Policy relating to privacy and confidentiality, as each <br />may now exist or be hereafter amended. <br />a. No access, disclosure or release of information regarding a child who is the <br />subject of Juvenile Court proceedings shall be permitted except as authorized. <br />If authorization is in doubt, no such information shall be released without the <br />written approval of a Judge of the Juvenile Court. <br />b. SAHA must receive prior written approval of the Juvenile Court before <br />allowing any child to be Interviewed, photographed or recorded by any <br />publication or organization orto appear on any radio, television or <br />internet broadcast or make any other public appearance. Such approval <br />shall be requested through child's social worker. <br />XIV. RETENTION OF RECORDS <br />SAHA, SSA and the CoC agree to retain all pertinent records, including, but not limited <br />to, client referrals, eligibility documentation, program reports, etc., in compliance with <br />federal, state and local laws or until all pending audits are completed, whichever is later. <br />XV. GENERAL PROVISIONS <br />1. With the exception of client records or other records referenced in Paragraph <br />VIII, entitled Confidentiality, all records, including, but not limited to, reports, <br />audits, notices, claims, statements and correspondence, required by this MOU <br />Pago 6 of 7 <br />3-9 <br />
