3. Effective January 1, 2022, the City shall contribute an allowance <br />towards the employee's Cafeteria Benefit plan for dental <br />insurance premiums for eligible employees and their qualified <br />dependents in the amount of one hundred ten dollars ($110) <br />per month per employee. <br />Any contribution necessary to maintain benefits under said dental plans in <br />excess of the amount of the City contribution to the cafeteria plan specified <br />above shall be borne entirely by the employee. <br />12.3 Vision Plan. The City shall maintain in effect its existing vision plan, for <br />employees covered by this MOU. All costs of the premium shall be paid by <br />the employee. <br />12.4 Long -Term Disability (L.T.D.) Insurance. The City shall continue to pay one <br />hundred percent (100%) of the premium cost fora 60-day elimination period <br />With a maximum benefit of $5,000 per month for a long-term disability <br />insurance plan for employees covered by this MOU. <br />Effective January 1, 2020, the City shall pay one hundred percent (100%) of <br />the premium cost for a 60-day elimination period with a maximum benefit of <br />$7,500 per month for a long-term disability insurance plan for employees <br />covered by this MOU. <br />12.5 Life Insurance. The City shall maintain in effect its existing life insurance plans <br />covering employees covered by this MOU, including term life insurance <br />coverage for each affected employee in the amount of twenty thousand <br />dollars ($20,000) plus twenty thousand dollars ($20,000) Accidental Death <br />and Dismemberment (AD&D) coverage at no cost to the employee. <br />A. Effective January 1, 2020, the term life insurance coverage for each <br />affected employee will increase to a maximum benefit amount of fifty <br />thousand dollars ($50,000) plus fifty thousand dollars ($50,000) <br />Accidental Death and Dismemberment (AD&D) coverage at no cost <br />to the employee. <br />12.6 The City shall retain the right to change health, dental and life insurance <br />carriers, administer the insurance benefits provided thereunder, and select <br />and/or change any excess or supplemental insurance carriers as a part of <br />any self-insurance plan during the term of this MOU, provided that <br />CONFIDENTIAL ASSOCIATION OF SANTA ANA (CASA) MOU: JANUARY I, 2019 THROUGH JUNE 30,2022 <br />Page 55 <br />55C-65 <br />
