• No additional compensation shall be paid for transporting the equipment <br />to or from the job site <br />(e) The Contractor shall submit weekly reports, when repairs are in progress, in a <br />format approved by Street Light Management, and shall be submitted (in <br />addition to hard copy) in Windows Excel spreadsheet. Weekly reports shall <br />include arrival and departure times, dates and addresses of the repairs, service <br />personnel dispatched, types of repairs made, and all materials and equipment <br />used for each day worked. Weekly reports shall be submitted by Monday the <br />following week. <br />(t) The Contractor shall provide to the Street Light Management Section As -Built <br />Maps of all work performed on a form approved by the City. As Builts shall <br />indicate the address and description of all repairs made including installation of <br />pull boxes, splices and connections, etc. As Builts shall be submitted following <br />each day's work performed. <br />(g) Emergency Services <br />Twenty-four hours per day, seven days per week, emergency service shall be <br />provided by Contractor upon City's call with a response time of less than two <br />hours under normal circumstances, to make immediate or temporary repairs to <br />City owned series and multiple street light pole or service knock downs or <br />damage caused by vehicle collision, acts of God, vandals, or as required because <br />of a public safety hazard. The Contractor shall respond to emergency calls when <br />directed by the City and dispatch high voltage technician and equipment. The <br />Contractor shall so conduct his operations as to cause the least possible <br />obstruction and inconvenience to public traffic. Contractor shall maintain and <br />make available to the City, a local telephone number where Contractor can be <br />contacted twenty-four hours per day. <br />I11t YiluTla":i`a:1DISi) RR <br />1. Street Light System Monthly Reports <br />Monthly reports shall include extraordinary maintenance repairs with As -built maps <br />for series street lighting. <br />2. Database for Maintenance Work <br />(a) At the City's Option, the Contractor will be required to utilize the City's <br />computerized system (Cityworks) to record ALL maintenance work performed on <br />the street light system. <br />(i) The City uses a computerized asset/work order system (Cityworks) to record <br />all work performed against City owned assets. The Cityworks system is a web <br />based application and available for download using an iOS or Android device. <br />(ii) The City will provide training and instruction to the Contractor on the use of <br />the Cityworks System. <br />(iii)The Contractor will be required to obtain a cellular/Wi-Fi enabled Apple <br />branded device to perform the required data entry in the field.
