EXHIBIT A <br />Appendix <br />ATTACHMENT 1 <br />Corr Or WORK <br />CITY OF SANTA ANA <br />REQUEST FOR PROPOSALS <br />FOR <br />STREET :LIGHT MAINTENANCE <br />REP NO.. 19-042 <br />i. PURPOSE <br />The City of Santa Ana is seeking an experienced contractor to perform preventative and <br />extraordinary Street Light maintenance services. <br />I1. STREET LIGHT MAINTENANCE <br />1. Scope Of We <br />Contractor shallfurnish all tools,, equipment, apparatus, facilities,; labor; services <br />and materials, and perform all work necessary to maintain street lighting facilities <br />in a good and professional manner in accordance with the spccifications gontained <br />h, MSlin. All said labor, services, materials, and equipment shall, be furnished and said <br />work performed and completed by the Contractor as an independent contractor, <br />subject to the inspection and approval of the City's Executive Director of Public <br />Works Agency, or inspectors or their representatives <br />2. Service Personnel <br />The Contractor shall provide qualified service personnel to maintain and to service <br />all series street lights and equipment described in Section 3. Work on the City's <br />series street lighting circuits shall be supervised and performed by a lead street <br />light technician with a minimum of five (5) years experience as a high voltage <br />technician. The technician shall troubleshoot, diagnose and perform all electrical <br />connections. <br />The Contractor shall include street lighting maintenance personnel's qualifications <br />in the proposal. During the term of the contract the City will notify the Contractor, <br />in the City's sole opinion, if personnel proposed for any maintenance are not <br />qualified to work on these: systems. <br />3. .Modifications to System <br />The City reserves the right to add or delete sections or circuits of Street Lights as <br />they are added or deleted from the City system. <br />4, Extraordinary Maintenance <br />(a) The failure or malfunction of street lights may be considered extraordinary <br />maintenance. <br />(b) Any upgrading, repair work, or any installation solicited by the City, shall be <br />considered as extraordinary.
