APPENDIX <br />ATTACHMENT I — SCOI <br />the City. Furthermore, City locations consist ofpublic-use facilities and recognizes the obligation <br />to ensure Contractor personnel and agents maintain the highest level of professional standards <br />in attire, decorum, and interaction with the public and City performeh Contractor's personnel <br />and agents shall comply with all City rules and regulations white on City premises. <br />Rules of Operation <br />I. Contractual Obligations: The City shall require certain contractual obligations, which <br />may include, but are not limited to, the following: <br />• Inclusion of Proposal: The proposal submitted in response to this RFP <br />shall be a required part of the final agreement with selected proposer. <br />fi. Agreement: A resulting agreement shall include any written material made as part of your <br />proposal. This material may include, but is not limited to: answers to questions contained <br />in this RFP, addenda, letters, telegrams, facsimile, and product literature. <br />Ill. Changes to Terms & Conditions: Proposers are to submit, as part of their proposal, any <br />changes or modifications to the terms and conditions they may wish the City to consider. <br />Proposers are encouraged to offer any other options, with the understanding that their <br />proposal shall first address the RFP requirements. Options are to be identified as such. <br />iv, Errors and Omissions. Proposer shall not be allowed to take advantage of any errors or <br />omissions in the RFP. Full instructions will be given if such error or omissionis <br />discovered and called to the attention to the City in a timely matnier, <br />5. UNIFORMS <br />Contractor personnel must wear uniforms supplied by the Contractor while on City premises. The <br />uniform will be subject to approval by the City and must consist of a long or short sleeved shut, full- <br />length pants, and have the Contractor's name and/or logo permanently affixed. <br />Contractor personnel must wear protective gear appropriate to the task(s) being performed, as <br />required by applicable Cal/OSHA regulations, <br />6. SUPPLIES and EQUIPMENT <br />The City will ONLY provide the following supplies: <br />Toilet paper Hand Towels <br />Toilet seat liners Trash bags <br />Hand soap <br />Supplies, equipment and tools which the Contractor must provide and maintain include, but are not <br />limited to: <br />City of Santa Ana RFP 19-074 <br />Attachment 1 (page 3 of 7) <br />25B-15 <br />
