ARTICLE XII <br />12.0 EMPLOYEE INSURANCE <br />12.1 Health Insurance. The City shall contribute the following amounts toward the <br />payment of premiums for affected employees and their dependents under <br />the existing health insurance programs or new program or programs <br />providing substantially similar or improved coverage and benefits selected in <br />accordance with procedures in effect on the effective date of this MOU: <br />A. For each such employee who is covered under a spouse's non -City <br />sponsored health plan, the City shall pay the employee a cash <br />payment each month in an amount equal to 50% of the monthly <br />premium amount for the City's lowest "employee -only" coverage, if <br />the employee waives, in writing, City -paid coverage. If an employee <br />waives City provided coverage, said employee shall provide proof of <br />medical insurance coverage in a non -City -sponsored health plan. <br />B. Beginning Jan 1, 2020: The City shall contribute an allowance towards <br />the employee's health insurance premiums for eligible employees and <br />their qualified dependents under The Public Employees' Medical & <br />Hospital Care Act (PEMHCA) which governs the California Public <br />Employees' Retirement System (CalPERS) Health Insurance Programs <br />pursuant to the provisions below: <br />Effective January 1, 2020, the City will provide a monthly <br />contribution to health insurance premiums at the following rates: <br />• Employee Only $704.00 <br />• Employee +1 $1,357.00 <br />• Employee + Family $1,750.00 <br />Effective January 1, 2021, the City will provide a monthly <br />contribution to health insurance premiums at the following rates: <br />• Employee Only $754.00 <br />• Employee +1 $1,407.00 <br />• Employee + Family $1,800.00 <br />CONFIDENTIAL ASSOCIATION OF SANTA ANA (CASA) MOU: JANUARY 1, 2019 THROUGH JUNE 30, 2022 <br />Page 53 <br />#10865v2 <br />
