3. Effective January 1, 2022, the City will provide a monthly <br />contribution to health insurance premiums at the following rates: <br />Employee Only <br />$804.00 <br />Employee +1 <br />$1,457.00 <br />Employee d- Family <br />$1,850.00 <br />C. Any contribution necessary to maintain benefits under City -sponsored <br />medical plans in excess of the amount set forth above shall be borne <br />entirely by the employee. <br />D. The parties recognize that certain State and Federal laws, programs <br />and regulations, including the Affordable Care Act, may impact <br />future medical plan offerings. In the event that reform measures <br />enacted during the term of this MOU alter healthcare options, cost or <br />other elements of healthcare services, thereby materially altering the <br />provisions of this MOU, the parties agree that upon the request of <br />either party the parties shall re -open Section 12.1 hereof regarding <br />medical insurance for the purpose of discussing alternative <br />approaches and proposals to providing healthcare benefits <br />hereunder. In addition, should State or Federal laws concerning the <br />taxation of healthcare benefits change, the parties agree to meet <br />and discuss the impact' of such change. <br />12.2 Dental Insurance. <br />A. The City agrees to contribute towards the premiums for dental insurance <br />plans provided by the City for employees covered by this MOU and their <br />eligible dependents on the following basis: <br />Effective January 1, 2020, the City shall contribute an allowance <br />towards the employee's Cafeteria Benefit plan for dental <br />insurance premiums for eligible employees and their qualified <br />dependents in the amount of one hundred dollars ($100) per <br />month per employee. <br />Effective January 1, 2021, the City shall contribute an allowance <br />towards the employee's Cafeteria Benefit plan for dental <br />insurance premiums for eligible employees and their qualified <br />CONFIDENTIAL ASSOCIATION OF SANTA ANA(CASA) MOU: JANUARY], 2019 THROUGH JUNE 30,2022 <br />Page 5h <br />#10SM5v2 <br />
