provided by the City for its employees and their eligible dependents in excess of the amount <br />of the City contribution to the Cafeteria Benefit Plait specified above shall be borne entirely <br />by the employee. <br />E. Cafeteria Benefit Plan. Effective January 1, 2020, the City provides a Cafeteria Benefit <br />Plan adopted in accordance with the provisions of Internal Revenue Code § 125 for <br />employees covered by this MOU. <br />Employees will be eligible to receive through the Cafeteria Benefit Plan, the cash <br />difference (subject to taxation as wages) if the monthly City allowance exceeds the <br />monthly cost of the Medical and Dental monthly plan premium cost. <br />For each such employee who is covered under a spouse's non -City sponsored health <br />plan, the City will pay the employee a cash -in -lieu payment (subject to taxation as <br />wages) each month in an amount equal to one hundred percent (100%) of the monthly <br />premium amount for the City's lowest "employee -only" coverage, if said employee <br />waives, in writing, City -paid coverage, said employee shall provide proof of medical <br />insurance coverage in a non -City -sponsored health plan. Said waiver shall include a <br />provision warning such employee that re-entry into any of the City -sponsored plans is <br />allowed only at open emollment, unless there is a qualifying event, and will require <br />proof of insurability for such employee and/or eligible dependents. <br />11.2 Dental Insurance. <br />A. The City shall continue to contribute the current maximum amount of $90 per <br />month per SAMA employee for employee dental insurance premiums. Effective January <br />1, 2016, the City contribution amount will be increased to a maximum of one hundred ten <br />dollars ($110) per month per employee. <br />B, Any contribution necessary to maintain benefits under said dental plans in excess <br />of the amount set fords above shall be borne entirely by the employee. <br />Effective January 1, 2020, the City agrees to contribute towards the Cafeteria Benefit plan <br />the payment of premiums for dental insurance plans provided by the City for employees <br />covered by this MOU and their eligible dependents on the following basis: <br />A. Effective January 1, 2020, the City shall contribute an allowance towards the <br />employee's Cafeteria Benefit plan for dental insurance premiums for eligible <br />employees and their qualified dependents in the amount of one hundred twenty dollars <br />($120) per month per employee. <br />B. Effective January 1, 2021, the City shall contribute an allowance towards the <br />SANTA ANA MANAGEMENT ASSOCIATION (SAMA) MOU:2019-2022 <br />
