employee's Cafeteria Benefit plan for dental insurance premiums for eligible <br />employees and their qualified dependents in the amount of one hundred twenty-five <br />dollars ($129) per month per employee. <br />C. Effective January 1, 2022, the City shall contribute an allowance towards the <br />employee's Cafeteria Benefit plan for dental insurance premiums for eligible <br />employees and their qualified dependents in the amount of one hundred thirty dollars <br />($130) per month per employee. <br />D. Any contribution necessary to maintain benefits under said dental plans in excess of <br />the amount of the City contribution to the cafeteria plan specified above shall be borne <br />entirely by the employee. <br />E. For each such employee who is covered under a spouse's non -City sponsored dental <br />plan, the City will pay the employee a cash -in -lieu payment (subject to taxation as <br />wages) each month in an amount equal to one -hundred percent (1000/0) of the City's <br />contribution, if the employee waives, in writing, City -paid coverage. Such waiver shall <br />include a provision warning such employee that reentry into any City -sponsored plan <br />is allowed only at open enrollment, unless there is a qualifying event, and may require <br />proof of insurability for such employee and/or eligible dependents. <br />11.3 Long -Term DisabilityLULD.l Insurance. <br />1. The City shall continue to pay one hundred percent (100%) of the premium cost for a 60- <br />day elimination period long term, disability insurance plan for employee covered by this <br />MOu. <br />2, Effective January 1, 2020, for employees covered by this MOU, the City shall continue to <br />pay one hundred percent (100%) of the premium cost for a 60-day elimination period long <br />term disability insurance plan with a maximum benefit of $10,000 per month for employee <br />only coverage. <br />11.4 Life Insurance. The City shall continue to pay one hundred percent (100%) of the premium <br />cost for teen life insurance coverage under the policy it maintains on behalf of employees <br />in order to provide employees covered by this MOU with life insurance coverage in an <br />amount equal to two times (2x) employee's annual rate of salary to a maximum of three <br />hundred thousand ($300,000) provided said employees can provide evidence of <br />SANTA ANA MANAGEMENT ASSOCIATION (SAMA) MOU: 2019-2022 0 <br />
