City of Santa Ana <br />I. Checklist of Forms to Accompany Proposal: <br />As a convenience to Proposers, the following is a list of the forms, included <br />as appendices to this RFP, which should be included with Proposals (aside <br />from the Statement of Qualifications requirement): <br />1. Non -Collusion Affidavit <br />2. Non -Lobbying Certification <br />3. Non -Discrimination Certification <br />4. Proposer's Statement <br />5. Staffing Plan <br />J. Insurance & W-9 Requirements: <br />Upon recommendation of contract award, Contractor will be required to <br />submit the following documents with ten (10) days of City notification, <br />unless otherwise specified in the solicitation: <br />Insurance - City requires that licensees, lessees, and vendors <br />have an approved Certificate of Insurance (not a declaration or <br />policy) or proof of legal self-insurance on file with the City for the <br />issuance of a permit or contract. <br />W-9 — Current signed form W-9 (Taxpayer Identification Number & <br />Certification) which includes Contractor's legal business name(s). <br />K. Licenses & Permits: <br />The selected Proposer shall be required to obtain a City of Santa Ana <br />Business license within ten (10) business days of selection and must provide <br />a copy to the City Project Manager or designee prior to commencing any <br />work in Santa Ana.
