8.3 Reports <br />8.3.1 Report Formats and Schedule <br />Records shall be maintained in forms and by methods that facilitate flexible use of data <br />contained in them to structure reports, as needed. Reports are intended to compile <br />recorded data into useful forms of information that can be used to, among other things: <br />a) Determine and set rates and evaluate the efficiency of operations; <br />b) Evaluate past and expected progress toward achieving AB 939 goals and <br />objectives; <br />c) Determine needs for adjustment to programs; and, <br />d) Evaluate Customer service and complaints. <br />Contractor may propose report formats that are responsive to the objectives and <br />audiences for each report. The format of each report requires approval by City. The <br />Contractor agrees to submit all reports by electronic means in a format compatible with <br />City's software/ computers at no additional charge, if requested by City. Contractor <br />will provide a certification statement, under penalty of perjury, by an authorized <br />Contractor official, that the report being submitted is true and correct. <br />Monthly reports shall be submitted within 30 calendar days after the end of each <br />month. Quarterly reports shall be submitted within 30 calendar days after the end of <br />the calendar quarter. If requested, Contractor's complaint summary, shall be sent to the <br />Executive Director within five days of request.
