a) Solid Waste Collected by Contractor for each month, sorted by type of Solid <br />Waste (Refuse, Recyclable and Organic Materials) and type of Customer <br />(Residential Curbside Service Unit, Multi -Family Premises Bin, Commercial <br />Premises or Roll -off) in tons, and the facilities where the tons were processed or <br />disposed. Bulky Waste items and used motor oil gallons and filters shall be <br />separately reported. This report is to be submitted in Microsoft Excel, or another <br />software as requested by City. <br />b) Warning notices issued for contaminated Recyclable Materials and Organics <br />Materials Containers. <br />c) Tons processed and recovered through Mixed Waste Processing. <br />d) HHW Collected. <br />e) If Contractor is directed by City to retain a consultant to provide public <br />education and outreach activities as required by Section 4.4.7, Contractor shall <br />submit consultant services performed and payments made by Contractor for the <br />preceding month and year-to-date. <br />f) Customer complaint logs as described in Section 5.2.2. <br />8.3.3 Quarterly Reports <br />The quarterly report should contain at a minimum the information required in the <br />monthly report and the following: <br />a) Copies of promotional and public education materials sent during the quarter. <br />b) Commercial recycling and organics site visits summary, including the name and <br />address of Customer, the date of the visit and the contact name and phone <br />number, demonstrating that the required visits have been made, and reason <br />provided for not establishing a recycling or organics program, in accordance <br />with Sections 4.3.5.1 and 4.3.6.1. The site visit summary will be completed on <br />forms approved by the City. <br />c) List of Customers that are required to participate in an Organics Recycling <br />program per Public Resources Code Section 42649.81. <br />d) List of Commercial and Multi -Family Premises Customers that do and do not <br />participate in an Organics program, whether the Organics program is provided <br />by -Contractor or another party, and whether the program is for Food Waste <br />March 9, 2020 105 City of Santa Ana - DRAFT <br />60A-194 <br />
