The DR Counselor shall attend FRC's Comprehensive Case <br />Management Team meetings. <br />3.5.3 <br />Human Options may offer <br />DR Individual <br />Counseling services <br />to all other FRC <br />participants who request <br />counseling services <br />provided that <br />all PARTICIPANTS <br />referred by ADMINISTRATOR <br />and DR Family <br />Advocate staff are <br />served as a priority <br />3.5.4 ADMINISTRATOR does not guarantee CONTRACTOR any specified <br />minimum number of referrals for DR Individual Counseling and acknowledges that <br />the number of PARTICIPANTS served is contingent in part, upon the number of <br />referrals to CONTRACTOR. <br />3.5.5 Human Options shall provide a minimum number of six (6) <br />and a maximum number of twelve (12) DR Individual Counseling sessions for each <br />PARTICIPANT during the term of the Agreement. Each counseling session shall <br />be a minimum of fifty (50) minutes in duration, or as clinically assessed by <br />the Clinical Supervisor. <br />3.5.6 Human Options shall provide DR Individual Counseling <br />services at the FRC, at PARTICIPANTS homes, in PARTICIPANTS' schools; and/or <br />other community sites as needed to assist in assessing family needs and <br />engaging PARTICIPANTS in counseling services. <br />3.5.7 Human Options shall measure progress ,by ensuring that <br />PARTICIPANTS complete a FaCT registration form. Additionally, Human Options <br />shall complete a FaCT standardized Counseling Assessment Tool and the FaCT <br />standardized Mental Health Counseling Indicators Checklist. <br />3.5.8 Human Options' DR Individual Counseling services shall <br />address the following PSSF service category: Family Preservation. <br />3.5.9 Human Options shall provide qualified DR Counselor and DR <br />Clinical Supervisor staff as specified in Subparagraphs 14.14 and 14.15 of <br />this Exhibit <br />(FNH0209) <br />11 of 42 <br />(3/18/09) <br />
