1 <br />2 <br />3 <br />4 <br />5 <br />6 <br />7 <br />8 <br />9 <br />10 <br />11 <br />12 <br />13 <br />14 <br />15 <br />16 <br />17 <br />18 <br />19 <br />20 <br />21 <br />22 <br />23 <br />24 <br />25 <br />26 <br />27 <br />28 <br />3,6 DR Family Counseling: <br />3.6.1 Human Options shall provide DR Family Counseling services <br />to at -risk families with children ages birth to eighteen (18) years of age in <br />the community who have been identified by ADMINISTRATOR and referred to <br />CONTRACTOR as potential risks for child abuse and/or neglect and/or referred <br />by DR Family Advocate staff, hereinafter referred to as "PARTICIPANTS" for <br />purposes of Subparagraph 3.6 of this Exhibit. <br />3.6.2 Human Options shall provide DR Family Counseling services <br />to a minimum of twelve (12) PARTICIPANTS. DR Family Counseling services shall <br />include, but not be limited to: assessing family needs; providing treatment <br />planning; providing emotional support; stabilizing immediate crisis; <br />increasing PARTICIPANTS' coping skills and family coherence; reducing <br />PARTICIPANTS' exposure to violence in the home; providing assistance <br />developing time -limited goals for the family and the child in placement that <br />are targeted to PARTICIPANTS' particular reunification plans. if applicable; <br />and, making appropriate linkages to all needed treatment programs and social <br />support systems. The DR Counselor shall attend FRC's Comprehensive Case <br />Management.Team meetings. <br />3.6,3 Human Options may offer DR Family Counseling services to <br />all other FRC participants who request counseling services provided that all <br />PARTICIPANTS referred by ADMINISTRATOR and DR Family Advocate staff are served <br />as a priority. <br />3.6.4 ADMINISTRATOR does not guarantee CONTRACTOR any specified <br />minimum number of referrals for DR Family Counseling and acknowledges that the <br />number of PARTICIPANTS served is contingent, -in part, upon the number of <br />referrals to CONTRACTOR. <br />3.6.5 Human Options shall provide a minimum number of six (6) <br />and a maximum number of twelve (12) DR Family Counseling sessions for each <br />(FNI10209) <br />12 of 42 <br />(3/18/09) <br />
