• His/her name, department name, and employee number; <br />• The number of hours of compensatory, holiday or vacation time to be <br />donated, within the limitations of Section 9. tOA(3); <br />• The name, department and employee number of the recipient; <br />• A statement indicating that the donor understands such donation of time <br />is irrevocable. <br />3. At the close of the donation period, the department shall verify that each <br />donating employee has accrued vacation, holiday, and/or compensatory time <br />balances sufficient to cover the designated donation. <br />The department shall submit all approved donation authorizations for an <br />eligible employee at one time for processing. No donation authorizations for <br />the eligible employee will be processed after this period. However, employees <br />who receive donations under this procedure and who exhaust all donated hours <br />may request an additional donation period subject to the provisions of <br />Section 9.1 OA. <br />The City shall add the donated time to the recipient's sick leave account.
