The City shall provide a separate payroll code for this deduction. <br />B. Remittance of Funds <br />The Union shall provide the City with an authorized deduction report each pay <br />period which includes a list of the employees who have authorized union <br />deductions and the amount of such deductions. <br />The City shall make the applicable deductions from the employee's paychecks and <br />electronically remit such funds to the Union via Electronic Funds Transfer (EFT) <br />following their deduction. <br />The City shall also send an itemized report, in Excel format, to the Union at <br />duesisseiu721.org. The report shall include a list of all union members in the <br />bargaining unit and include the following information on each union member: first <br />name, last name, employee identification number, employment status (i.e. active or <br />on an unpaid leave of absence), base salary earned in the pay period, salary step, <br />total hours in a paid status for the pay period, and the amount remitted on behalf of <br />the employee. <br />The remittance of funds and itemized report shall be sent to the Union semi- <br />monthly. <br />C. Maintenance of Dues Payroll Deduction <br />Any employee who has authorized union dues deductions on the effective date of <br />this MOU, or at any time subsequent to the effective date of this MOU, shall <br />continue to have such dues deduction made by the City during the term of this <br />MOU; provided, however, that any employee may revoke such dues deductions in <br />the last twelve ( l2) months of this MOU during the twenty-one (21) calendar day <br />period preceding the employee's date of hire anniversary, by notifying the Union <br />of their revocation of union dues deductions. Such notification shall be in writing <br />and contain the following information: employee name, employee identification <br />number, job classification, department name, bargaining unit, and name of the <br />employer from which such dues deductions are to be cancelled. Upon request, the <br />Union will inform the union member of his/her dues deduction revocation date. The <br />Union will provide the City with the appropriate documentation to process these <br />dues revocations within ten (10) business days after the close of the withdrawal <br />period. <br />D. Indemnification <br />The Union shall indemnify and hold the City, its officers and employees, harmless <br />from any and all claims, demands, suits, or any other action arising from this <br />section.
