Affordable, accessible housing: Special needs populations comprise a disproportionate share of the low - <br />and moderate -income population within La Habra and, as such, have a greater need for affordable <br />housing — including accessible housing for elderly and disabled households. <br />Accessible public facilities: Elderly and disabled individuals require public facilities and services to be ADA <br />compliant to enable them to effectively access services and programs throughout the City. <br />Public services: Special needs populations require supportive services such as transportation, education, <br />senior services, childcare and basic needs. <br />Describe programs for ensuring that persons returning from mental and physical health <br />institutions receive appropriate supportive housing <br />Within Orange County, several service providers operate county -wide services to provide mental and <br />physical health counseling and supportive services. <br />The Mental Health Association of Orange County provides transition support to individuals with mental <br />health challenges and issues. Support includes identifying supportive housing as well as community <br />resources and other services to meet the individual's needs. <br />The Dayle Macintosh Program provides transition support in Orange County to people with disabilities <br />who reside in long-term institutions and have a desire to live independently. Clients are paired with a <br />transition coordinator who helps identify supportive housing as well as other needs and services to enable <br />the individual to successfully live independently. <br />For entitlement/consortia grantees: Specify the activities that the jurisdiction plans to <br />undertake during the next year to address the housing and supportive services needs identified <br />in accordance with 91.215(e) with respect to persons who are not homeless but have other <br />special needs. (91.220(2)) <br />Through the City's public service activities, the City will prioritize funding to address supportive service <br />needs of vulnerable and special needs populations in the City, including (elderly, youth, disabled, victims <br />of domestic violence). These activities will fund a range of services includingjob training, legal assistance, <br />supportive services, victim intervention, health care, and basic services and needs. <br />The City will continue to prioritize the preservation and expansion of affordable housingthrough the City. <br />In PY2020, Santa Ana anticipates that it will allocate over $3.6M to support and promote affordable <br />housing within the City.
